ROCK FELL FROM BETWEEN ROOFBOLTS STRIKING MINER ON HEAD CAUSING HIM TO FALL FORWARD BUMPING FRONT OF HEAD ON CANOPY OF ROOF BOLT MACHINE. ROCK FELL FROM AN APPROX. DISTANCE OF 1 FOOT ABOVE HEA D MEASURING 24' X 50' X 3". REACTION TO BEING PUSHED FORWARD AND DOWN CAUSED MINER TO RAISE QUICKLY BUMPING AND CUTTING HEAD ON TOP.
