The injured employee was on the ground, as a rigger for the crane operator. EE and another employee were hooking up the crane cables to the crusher operator building platform. The injured employee stepped up onto the platform to grab the hook, when EE stepped through a hole in the platform. The edge of the expanded metal deck caused a laceration to EE's R/ shin.

EMPLOYEE WAS SWINGING SLEDGE HAMMER TO POUND IN A WEDGE ON THE LOCKOUT POST ON CONE CRUSHER. HIS KNEE BUCKELD WHILE HE WAS SWINGING. THEN ON 10-13-94 EMPLOYEE WAS WALKING UP STEPS AT JADAIR CL ARIFIER AND HIS KNEE BLEW OUT. HE WILL BE GETTING IT OPERATED ON 10-31-94. THERE WILL BE LOST TIME AFTER THE OPERATION.

OPERATOR WAS MAKING R-4 RIP RAP WITH 966B LOADER WHEN THE LOADER'S HYDRAULIC FOR STEEING SYSTEM STARTED A JERKING MOTION SIDE TO SIDE WHICH IN TURN CAUSED EMPLOYEE'S NECK TO SNAP SIDE TO SIDE CAUSING DISCOMFORT.

EE WAS OPERATING A NORTHWEST 80D SHOVEL. WHILE OPERATING THE UNIT. THE RIGHT BRAKE PEDDLE SNAPPED BACK UNEXPECTEDLY THIS RESULTED IN PULLED RIGHT CALF MUSCLES.

EMPLOYEE WAS PICKING UP A SMALL PIECE OF STEEL IN THE SHOP WHEN HE FELT A SLIGHT PAIN IN HIS LEFT ARM. THE NEXT DAY HE WENT TO THE DOCTOR AND WAS TOLD THAT HE TODE THE LIGAMENT IN HIS LEFT BIC EP.
EMPLOYEE WAS BACKING UP A D-8 BULLDOZER TRYING TO AVOID A THREE FEET HIGH BENCH OF STONE. THE EMPLOYEE MISJUDGED AND BACKED INTO THE BENCH CAUSING THE BULLDOZER TO STOP SUDDENLY. THE EMPLOYEE' S BACK AND NECK WERE WRENCHED.
