Employee did not follow training and approached a slope with bucket down. Machine slid down incline. Employee was not wearing safety belt and hit windshield. Machine not affected.

The control office was struck by lightning during off shift hours. The plant was not in operation at the time of accident. The plant was not scheduled to operate on the weekend thus no persons were at the control office during the time of accident. Fire was discovered and 911 call placed. Local fire department extinguished.
