Carr Brothers & Sons, Inc. · Fall to the walkway or working surface

Miner was engaged in maintenance activities with the witness. Witness had handed employee a rubber rail which EE threw down. EE then grabbed a loose metal rail which shifted and the witness heard a thud. The witness looked onto the catwalk and saw the injured employee laying on the catwalk with hand on the back of head but did not see how the injury was incurred.
