Employee was getting off of the truck when EE slipped on slick material spraining the ankle. After visiting the doctor EE was given a brace to wear.

Getting out of a stock pile truck. Pushed down on the door handle, breaking the handle and lacerating the left hand on the broken piece.

Dump area was getting undercut. Loader operator warned truck driver about situation and no response. Next load brought up, he backed up too far and the pile gave way and the truck slid down the material and flipped over onto its top. Truck operator was not wearing a seatbelt.

EMPLOYEE WAS REMOVING A TARP STRAP THAT WAS HOLDING ELECTRICAL CORDS; STRAP SNAPPED BACK AND STRUCK HIM IN THE CHIN, CAUSING A LACERATION REQUIRING STITCHES.

DRILLER WAS REMOVING DRILL BIT FROM BROKEN DRILLSTEEL. THE BALL PEEN HAMMER HE WAS USING CHIPPEDAND THE CHIP LODGED IN THE DRILLER'S CHEST. THE CHIP WAS LATER REMOVED BY A DOCTOR.

EMPLOYEE WAS USING A PRY BAR TO START A WIPER BACK WITHIN A CONVEYOR WHILE IT WAS IN OPERATION. THE MOVING BELT CAUGHT THE BELT, PULLED IT FROM THE EMPLOYEE'S HAND AND GASHED HIS RIGHT FOREARM .
WHILE DRILL WAS RUNNING THE NEW HIR OPERATOR REACHED UP HIGH ON THE DRILL STEEL AND CLOSED HIS RIGHT HAND AROUND IT. THE DRILL STEEL CAUGHT HIS GLOVE AND NEARLY WRAPPED HIM UP IN IT BEFORE HE COULD SHUT OFF THE DRILL.
