DRIVER WAS DUMPING LIMESTONE FROM TRUCK WHICH WAS ON LEVEL GROUND IN YARD STOCKPILE. DRIVER WAS IN THE PROCESS OF DUMPING HIS LOAD OF STONE WHEN THE TRACTOR AND TRAILER TIPPED OVER ON THE PASS ENGER SIDE. DRIVER LANDED ON GROUND CLEAR OF TRUCK. DRIVER WAS KNOCKED UNCONSIOUS BY THE IMPACT OF THE FALL, BUT LATER REGAINED CONSCIOUSNESS. NO BLEEDING WAS NOTICED. DRIVER WAS LIFEFLIGHTED

EE WAS ASSISTING IN REPLACING A REBUILT JAW CRUSHER ASSEMBLY JAW DIE INTO THE MAIN PLANT CRUSHER. AN EYE WAS WELDED ONTO THE JAW DIE, ENABLING IT TO BE LIFTED. THE JAW DIE WAS BEING LIFTED BY THE WELDED ON EYE WHEN THE WELD BROKE AND THE JAW DIE FELL TWO FEET TO THE GROUND LANDING ON THE END OF THE EE S RIGHT FOOT. THE EE WAS DRIVEN TO THE HOSPITAL BY ANOTHER EE X RAY REVEALED BROK
