Employee was working on a ladder approx. 3 ft. off the ground when EE fell and broke a bone in EE's lower right leg near EE's ankle.

The alleged injured employee stated that he was lifting on a screen bearing in order to remove from the crane choker. He stated that he felt a pain in his lower back. It was later determined through interviews and investigation that the injury was not a work related injury.
The employee was using a knife to cut a limb on a tree. As the employee was cutting the limb the knife slipped causing a laceration that required stitches.

Injured employee had installed a bearing puller, and was using a porta-power to reverse the bearing from the shaft. When one of the all thread stock stripped allowing the T-Bar and the all thread rods to come apart under pressure causing a laceration and fracture to thumb

Local police were in pursuit of an automobile on Hummingbird Lane, in front of mine property. The driver swerved to avoid police spike strips, lost control and ran through fence near Vulcan's front gate, hitting a tree resulting in a fatal car crash 20 feet onto mine property.

THE INJURY OCCURRED WHILE TWO EMPLOYEES' WERE ATTEMPTING TO RIG FOR REMOVAL OF THE CRUSHER COVER. THE SINGLE PART LINE WAS TWO-BLOCKED ALLOWING THE HOOK & HEADACHE BALL TO DROP, STRIKING THE EE THAT WAS IN CLOSE PROXIMITY TO SET RIGGING.
THE EMPLOYEE WAS AT THE SCREEN HOUSE CLEANING OUT AND ADJUSTING THE WATER NOZZLES IN FRONT OF THE SCREEN. HE TURNED TO EXIT THE AREA AND HIT HIS LEG ON A BEAM, RESULTING IN STITCHES TO HIS RIG HT LEG.
