Double Mountain Mine: 1 recorded fatality Coal

The roof bolter operator was loading metal straps onto the roof bolting machine. As the employee was loading the metal straps their hand started to slip and the metal straps shifted or slid and the corner of the metal strap cut the employee on the inside of the right forearm.

The injured employee was on EE's knees hanging the roof bolter cable slack. After the cable was hung, the operator started to get onto the roof bolter. The operators hand slipped and EE pushed down on the automatic cable reel take up. The injured EE was close to the cable and when the reel engaged the cable became tight and struck EE pushing EE to the side, twisting EE's left knee.

The injured employee was attempting to remove a cord lash or nylon band from a bundle of roof bolts. The employee was using a utility or razor knife to cut the band on the bundle of bolts. As the employee used left hand to cut the banding material, the knife slipped and cut right index finger.

The injured employee and Co-worker was removing the slack from the conveyor chain of a continuous miner. The employee had removed links from the conveyor chain and was re-assembling the pins and keepers in the chain. A sledge hammer was being used to drive the lock rings onto the pins when the co-worker hit the injured employee's left little finger with the sledge hammer.

The employee was removing a roll of brattice cloth from the front of the section mantrip. As the employee picked up the roll of brattice cloth, he started to twist and felt a sharp pain in the lower portion of his back.

Employee was pre-shifting belts on 002 section panel belt on #3 entry break #2. While walking his left ear came in contact with a metal strap cutting his left ear, requiring stitches.

The injured employee and co-workers were removing wooden timbers from the bucket of a scoop. The employee was down on one knee pulling the wooden timber out of the bucket when he felt a sharp pain in the lower right side of his back and right hip.

A no injury roof fall occurred at the no. 1 mine portal. The roof fall damaged the mine canopy at the entrance to the no. 1 entry. The roof fall was rock from the high wall and the edge of the mine roof for approximately 4'-6' feet underground. Upon discovery of the roof fall, MSHA was notified. The area was cleaned up and a new canopy was installed in the no. 1 portal.

Employee was in the No.3 entry and started to pull an electrical cable away from the low belt structure when he felt a sharp pain in his right side near the groin area.

The employee was loading supplies onto a roof bolting machine. While lifting the box of resin glue he twisted and felt a sharp pain on the inside of his right leg and right side.

The employee was working on a belt head drive removing structure off the head drive when one of the rollers pinched the left index finger which resulted in a small laceration and a open fracture to the left index finger.

A Roof Fall was found during a weekly examination. The Roof Fall is located on the 1 West Panel off the 1 South Mains. The roof fall is approximately 50' ft. long x 18.5' ft. wide x 6' ft. thick. The roof fall did not affect the mine ventilation or impede passage. The roof fall is located in the outby area of the mine. There are no injuries associated with this roof fall.

Employee was stepping out of the man basket to get extension cord twisting his knee. "Left Knee"

Employee was changing a torque shaft on the gathering head of the 14-15 miner. When he pulled the broke shaft out it had built up pressure and it sprayed hot oil out striking him on the neck and face.

Employee was shoveling loose coal and rock up, when he felt a sharp pain in his left knee and couldn't put weight on it.

Employee was moving leaky feeder cable up. When he crossed a mobile bridge he said his knees buckled when he put them on the bottom. Also he said they had been hurting for a couple of weeks

The employee was moving belt structure and picked up a belt rail and twisted around to put it in a stand, pulling a muscle in his chest. He is not sure what he done.

Victim was riding a mac-8 mantrip buggy. Buggy turned into a cross-cut at the track entry and a bundle of roof straps was laying on rib. Victims foot was caught and twisted it around hurting his right knee.

Employee was helping hang a miner cable, and it fell and jerked his back. His doctor said it had slipped a disk.

While loading timbers in scoop bucket, one was standing up, and when the employee started to lay it down, it took him to his knees and hurting his back.

EE was reaching for a roof bolt plate and his right arm hit a metal strap cutting his arm just below his elbow. After going to the doctor he received 5 stitches in his arm.

EE was pulling a timber out of scoop bucket when he pulled a muscle in his left arm around the elbow.

Injured jumped down approximately 3' to ground twisting his right knee.

Weathering due to freezing & thawing. Had to move life line over into #3 entry, while rock was cleaned up and top supported. After clean up life line was re-run back to #4 belt entry.

The employee was working on the miner, he was lifting a lid up and pulled a muscle in his lower back.

EE was roof bolting while drilling a hole his wrench bent allowing the drill steel to come out and hit the EE on the right arm. Above the wrist causing a knot to come up on it.

EE was stocking shelves in the warehouse and was lifting a box of parts when he felt a muscle pull in his stomach. Later in the shift a knot appeared on stomach.

He was helping co-worker load garbage in scoop bucket, while pulling on bent roof bolts one was hung and when it came loose struck him on the right side of the mouth chipping 2 teeth.

Victim was walking to get on Damascus buggy to come outside, he pulled a muscle in his right hip.

Employee was installing a roof bolt and bent it. While trying to straighten it, he hit the rotation lever and spun the bolt twisting left wrist.

Miner was walking around roof bolter when his arm touched the canopy of machine. A burr on the canopy cut his right arm.

A piece of draw rock fell cutting his left arm, which required stitches.

Tripped and fell into machine. A strap on the machine was hanging off the side, which cut his ear.

Victim was loading bolting supplies on roof-bolter he turned his body while holding 5 roof bolts and hurt his lower back.

Victim was cleaning canopy off. He was cleaning loose debrie off. He reached down and lifted a piece of wire mesh to remove the dirt and pulled muscle in back.

Employee was installing roof bolts, he bent one and was trying to get it back out of the hole. He was using the head to pull it back out of the hole, he hit the wrong lever and spun the bolt, twisting his right wrist. And after xrays was read, he has a fracture in his wrist.
