Non-injury roof fall was found in 5 left panel off 4-North Mains. The roof fall was found during a weekly examination. Roof fall begins in No. 5 entry of 5 left panel and extends inby to crosscut No. 12. Roof fall then continued into No. 6 entry of 5 left panel. Roof fall appears to be approx. 4ft to 8 ft. thick and approx. 18 to 20 feet wide (see map). MSHA Hotline notified.

Employee was raising the boom on the belt-head drive. EE was twisting screw jack on the belt drive when EE slipped and fell twisting EE's right knee. The accident occurred on 8/22/17. The EE returned to regular duties after the accident and continued to work until EE's shift on 8/25/17 - 8/26/17.

A Non-Injury Roof Fall occurred on 2/27/17. The roof fall is located in the No. 3 entry at x-cut No. 15 on the 2-Left Panel off the 4th North Mains. The approx size of the roof fall is 40' in length, 18'-19' wide x 6'-8' thick. The MSHA Hot line was notified. Call conf. No. 3100034. The fall has been dangered off/wooded cribs installed on all sides of roof fall.

The roof bolter operator was loading metal straps onto the roof bolting machine. As the employee was loading the metal straps their hand started to slip and the metal straps shifted or slid and the corner of the metal strap cut the employee on the inside of the right forearm.

The injured employee was on EE's knees hanging the roof bolter cable slack. After the cable was hung, the operator started to get onto the roof bolter. The operators hand slipped and EE pushed down on the automatic cable reel take up. The injured EE was close to the cable and when the reel engaged the cable became tight and struck EE pushing EE to the side, twisting EE's left knee.

The injured employee was attempting to remove a cord lash or nylon band from a bundle of roof bolts. The employee was using a utility or razor knife to cut the band on the bundle of bolts. As the employee used left hand to cut the banding material, the knife slipped and cut right index finger.

The injured employee and Co-worker was removing the slack from the conveyor chain of a continuous miner. The employee had removed links from the conveyor chain and was re-assembling the pins and keepers in the chain. A sledge hammer was being used to drive the lock rings onto the pins when the co-worker hit the injured employee's left little finger with the sledge hammer.

The injured employee and co-workers were removing wooden timbers from the bucket of a scoop. The employee was down on one knee pulling the wooden timber out of the bucket when he felt a sharp pain in the lower right side of his back and right hip.

A no injury roof fall occurred at the no. 1 mine portal. The roof fall damaged the mine canopy at the entrance to the no. 1 entry. The roof fall was rock from the high wall and the edge of the mine roof for approximately 4'-6' feet underground. Upon discovery of the roof fall, MSHA was notified. The area was cleaned up and a new canopy was installed in the no. 1 portal.

The roof fall did not affect the mine ventilation or impede passage. The roof fall is located in the outby area of the mine. There are no injuries associated with this roof fall.

Employee was installing roof bolts, he bent one and was trying to get it back out of the hole. He was using the head to pull it back out of the hole, he hit the wrong lever and spun the bolt, twisting his right wrist. And after xrays was read, he has a fracture in his wrist.
