SWINGJACK FRAME WAS BROKE ON ROOF BOLTER THE DRILL HEAD OF THE BOLTER WAS PULLED AWAY FROM THE BOLTER AS EE & ANOTHER EE WERE PUSHING THE HEAD BACK TOWARD THE BOLTER INJ EE FELT A SHARP PAIN IN HIS CHEST AREA. HE WORKED THAT SHIFT & WORKEDMONDAY 2/22/99 WHEN THE PAIN GOT WORSE. HE WENT TO DR ON MON NIGHT AND WAS ADMITTED TO HOSPITAL
