ACCIDENT HAPPEN9D IN THE #3 ENTRY. AS BOLTING MACHINE WAS BEING BACKED OUT OF #3 ENTRY, THE CANOPY OVER OPERATOR'S DOCK "HUNG" ON A ROOFBOLT. WHEN THIS HAPPENED THE BOLTING MACHINE JERKED. TH E INJURED WAS W5LKING BESIDE MANCHINE AND WHEN MACHINE JERKED, A 36" ROOFBOLT CAME OUT OF SUPPLY TRAY AND HIT INJURED UNDER RIGHT EYE CAUSING A LACERATION WHICH REQUIRED 4 SUTURES.
