Employee was descending a ladder following changing a light on a front end loader. Accidently stepped off the second rung and fell. This resulted in a back contusion and restricted duty from occupational doctor.

Secondary plant operator had several pieces of metal embedded into left eye during the shift. The metal most likely came from EE's gloves. Metal was removed and an antibiotic was administered making this a reportable injury.

While scaling, EE was jarred in the seat when the rock the EE was scaling broke free. EE began having pain in back and reported it to supervisor. Resulted in a back strain. No deficiencies found with seat.

While leaving for the day the scale operator fell on her way to her car. There is a step down from the sidewalk to the parking lot of about 3 inches. When she stepped she caught this drop mid foot which then led to her falling on her left shoulder. At this point no damage has been identified but she is on a lifting restriction with use of a sling.

While using water to spray out built up material in a chute the employee received a splash of water and lime material in the face. Received abrasion and prescribed meds, employee cleared to work with no restriction. PPE was being worn - hardhat, safety glasses, etc.

While working on the crusher feeder employee slipped, pinched his finger between cap he was holding and chute work. Employee failed to report the injury until the next day after seeing his personal doctor, who treated it as a first aid. On 9/17/04 we received notification of a fracture to the tip of his finger, which made this a reportable injury.

The EE was instructed to push in the 3I surge pile. the EE exited his loader to remove cones that were in front of the syntron hole. when the EE went to move the cones he slipped on ice/snow that was in the yard catching himself with his left shoulder.

EE was in the underground shop working on the scaler, EE tripped on the mono-rail and fell to the ground. EE landed on his left knee cause the fracture to the patella.

While exiting the cab of the drill the operator was looking back at the engine compartment checking the AC unit which was worked on earlier that morning. At this point the EE said when he twisted to look in this area his knee gave out and he lunged forward catching himself with his right arm on the door jamb (EE was standing on the ground when knee gave out)

While shoveling out a corner on Friday, the EE nicked his knuckle on a rock. EE did have gloves on and it did not cut the gloves. EE did not realize he cut his finger until he removed his gloves. When the EE arrived to work on 12/21/10, his finger was swollen and had a fair amount of pain. EE was set up with a doctor appointment and he was administered an antibiotic shot.

Technician installing a wiring harness on a 248 skid loader. The cab was raised supported by the cab safety lock and the hoist. Working conditions were normal, no slippery materials that would have caused the slip. While backing out of the belly left foot slipped he fell causing a cut on his head required seven stitches.

EE suffered a non-displaced fracture of tip of his left ring finger. EE was installing a new throat bracket on a 50" hammer mill. EE was wearing 2 pairs of gloves but had his finger in a pinch point as the bracket was being installed.

At the end of shift as the driller was backing his drill away from a heading, he noticed a fire. He exited the machine and found the flames too intense to activate the onboard fire suppression system. He ran to an equipment parking area, sounded the fire alarm, the mine was evacuated without incident or injury. MSHA one call was initiated, the mine was barracaded and attended.

Employee was operating a three ton come-a-long tightening a belt for a belt splice. A chain on one of the belt clamps failed and the come-a-long flew back and hit the employee on his left leg. The impact caused his left foot to turn inward adn fracture his left ankle. This incident initially was a medical and as of 10/15/08 has become a lost time injury.

Contractor was repairing torque converter output seal, laying on right side platform with his legs extended over the platform. He used his abdomen muscles to arch up to reach back for a wrench and popping noise occurred in right knee. He climbed off machine and walked around trying to stretch and maneuver his knee. That is when the real pain started.

EE claimed to have injured his back on 11/4/05 while holding a 15" bar in place against the #4 belt. EE received medical attention and eventually surgery before notifying management on 12/13/05 that it was work-related. Employer and insurance administrator determined to accept claim on 1/24/06. EE had identical injury in 2005 which was not work-related.

THE SCALER OPERATOR WAS INSTRUCTED TO BOOM DOWN TO CREAT MORE SLACK TO ALLOW THE CHAIN TO BE TIGHTENED AROUND THE BAR. THE SCALER HAS A FLOATATION VALVE THAT RELEASED BELOW A CERTAIN ELEVATION ALLOWS THE BOOM TO GO TO THE FLOOR. WHEN THE BOOM DESCENDED IT CAUGHT THE EE'S HAND.

A CO-WORKER MOVED THE VEHICLE AHEAD CAUSING THE INJURED EMPLOYEE TO FALL OV ER THE END GATE LANDING ON HIS OUTSTRETCHED HANDS. THE FALL WAS APPROX 4' CAUSING MULTIPLE FRACTURS OF THE LEFT ARM BELOW THE ELBOW.
