Durham Mine & Mill: 424 MSHA citations Metal/Non-Metal

Employee was descending a ladder following changing a light on a front end loader. Accidently stepped off the second rung and fell. This resulted in a back contusion and restricted duty from occupational doctor.

Secondary plant operator had several pieces of metal embedded into left eye during the shift. The metal most likely came from EE's gloves. Metal was removed and an antibiotic was administered making this a reportable injury.

The employee was changing screens in the wash deck. The screen slid down cutting the wrist of employee.

Employee's leg was caught in between powder machine and rib resulting in laceration to calf area of left leg. Employee was taken to ER and received stitches.

While scaling, EE was jarred in the seat when the rock the EE was scaling broke free. EE began having pain in back and reported it to supervisor. Resulted in a back strain. No deficiencies found with seat.

While leaving for the day the scale operator fell on her way to her car. There is a step down from the sidewalk to the parking lot of about 3 inches. When she stepped she caught this drop mid foot which then led to her falling on her left shoulder. At this point no damage has been identified but she is on a lifting restriction with use of a sling.

While using water to spray out built up material in a chute the employee received a splash of water and lime material in the face. Received abrasion and prescribed meds, employee cleared to work with no restriction. PPE was being worn - hardhat, safety glasses, etc.

While working on the crusher feeder employee slipped, pinched his finger between cap he was holding and chute work. Employee failed to report the injury until the next day after seeing his personal doctor, who treated it as a first aid. On 9/17/04 we received notification of a fracture to the tip of his finger, which made this a reportable injury.

The EE was instructed to push in the 3I surge pile. the EE exited his loader to remove cones that were in front of the syntron hole. when the EE went to move the cones he slipped on ice/snow that was in the yard catching himself with his left shoulder.

EE was in the underground shop working on the scaler, EE tripped on the mono-rail and fell to the ground. EE landed on his left knee cause the fracture to the patella.

While exiting the cab of the drill the operator was looking back at the engine compartment checking the AC unit which was worked on earlier that morning. At this point the EE said when he twisted to look in this area his knee gave out and he lunged forward catching himself with his right arm on the door jamb (EE was standing on the ground when knee gave out)

While shoveling out a corner on Friday, the EE nicked his knuckle on a rock. EE did have gloves on and it did not cut the gloves. EE did not realize he cut his finger until he removed his gloves. When the EE arrived to work on 12/21/10, his finger was swollen and had a fair amount of pain. EE was set up with a doctor appointment and he was administered an antibiotic shot.

Technician installing a wiring harness on a 248 skid loader. The cab was raised supported by the cab safety lock and the hoist. Working conditions were normal, no slippery materials that would have caused the slip. While backing out of the belly left foot slipped he fell causing a cut on his head required seven stitches.

EE suffered a non-displaced fracture of tip of his left ring finger. EE was installing a new throat bracket on a 50" hammer mill. EE was wearing 2 pairs of gloves but had his finger in a pinch point as the bracket was being installed.

At the end of shift as the driller was backing his drill away from a heading, he noticed a fire. He exited the machine and found the flames too intense to activate the onboard fire suppression system. He ran to an equipment parking area, sounded the fire alarm, the mine was evacuated without incident or injury. MSHA one call was initiated, the mine was barracaded and attended.

Employee was operating a three ton come-a-long tightening a belt for a belt splice. A chain on one of the belt clamps failed and the come-a-long flew back and hit the employee on his left leg. The impact caused his left foot to turn inward adn fracture his left ankle. This incident initially was a medical and as of 10/15/08 has become a lost time injury.

Contractor was repairing torque converter output seal, laying on right side platform with his legs extended over the platform. He used his abdomen muscles to arch up to reach back for a wrench and popping noise occurred in right knee. He climbed off machine and walked around trying to stretch and maneuver his knee. That is when the real pain started.

EE claimed to have injured his back on 11/4/05 while holding a 15" bar in place against the #4 belt. EE received medical attention and eventually surgery before notifying management on 12/13/05 that it was work-related. Employer and insurance administrator determined to accept claim on 1/24/06. EE had identical injury in 2005 which was not work-related.

Employee was putting left side track on a Cat D3 dozer back together. He was installing a new track pin and struck it with a hammer. A piece of the pin broke off and imbedded in the employees upper left thigh.

ON THE LAST DAY SHE WORKED THE EMPLOYEE LEFT THE PROPERTY WITHOUT REPORTING ANY TYPE OF INJURY TO PLANT MANAGEMENT.

EE WAS MOVING FORKLIFT TINES AND PINCHED INDEX FINGER BETWEEN TINE AND SUPPORTING STRUCTURE.

EE WERE LIFTING A BAR TO BE INSTALLED INTO THE PRIMARY CRUSHER UNDERGROUND USING A CHAIN & SCALER. THE SCALER OPERATOR WAS INSTRUCTED TO BOOM DOWN TO CREAT MORE SLACK TO ALLOW THE CHAIN TO BE TIGHTENED AROUND THE BAR. THE SCALER HAS A FLOATATION VALVE THAT RELEASED BELOW A CERTAIN ELEVATION ALLOWS THE BOOM TO GO TO THE FLOOR. WHEN THE BOOM DESCENDED IT CAUGHT THE EE'S HAND.

EE'S WERE LIFTING A BAR TO BE INSTALLED INTO THEPRIMARY CRUSHER UNDERGROUND USING A CHAIN AND THE SCALER. THE SCALER OPERATOR WAS INSTRUCTED TO BOOM DOWN TO CREATE MORE SLACK IN THE CHAIN ALLO WING THE CHAIN TO BE TIGHTENED SLIGHTLY AROUND THE BAR. THE SCALER HAS A FLOATATION VALVE THAT RELEASED BELOW A CERTAIN ELEVATION ALLOWING BOOMTO GO TO FLOOR. CAUGHT EE HAND BETWEEN BAR& BOOM

3 EE'S WERE REPLAICNG HAMMER TIPS ONT HE CEDAR RAPIDS HAMMER MILL. EE WAS SITTING ON TOP OF THE OPEN LID OF THE MILL WHILE ANOTHER EE WAS ROTATING THE MILL BY HAND USING A LARGE PIPE WRENCH ON T HE FLY WHEEL. THE HAMMERS WERE AT THE TOP OF THE ROTATION AND THE WRENCH WAS BEING REPOSITIONED WHENT HE HAMMERS FELL OVER CENTER AND ROTATED BACK ONTO EE S FOOT THAT WAS STICKING OUT OVER

EMPLOYEE WAS ATTEMPTING TO SECURE A SMALL CONVEX MIRROR WITH A PAIR OF VICE GRIPS ONTO A SUPPORTING LEG OF AN OVERHEAD BIN. FOOTPRINTS FOUND AT THE SCENE SHOW THAT HE WAS STANDING WITH A FOOT ON A SUPPORTING ANGLE AND THE OTHER AT THE BASE OF THE SUPPORTING ANGLE AND THE VERTICAL I-BEAM.

EE WAS REMOVING A DRIVE GEAR @ HEAD SECTION OF UNDERGROUND BELTLINE, WHEN THE GEAR SLIPPED OUT OF THE EE'S HAND CAUSING HIS RIGHT THUMB TO BE LACERATED.

DECLINE IS CONSTRUCTED OF AN 18' STEEL TUBE SET PARALLEL AT APPROX 15' THE TUBE THAT SERVES AS THE SECONDARY ESCAPE ROUTE AND HOUSES THE BELTLINE BRINGING PRODUCT OUT OF THE MINE WAS PARTIALLY BEING COVERED BY REJECT LIME BEING BACK FILLED OVER THE TUBE. THE TUBE COLLAPSED CLOSING OFF THE BELTLINE. MINE WAS EVACUATED AND MSHA NOTIFIED. NE PERSONS OR EQUIPMENT IN AREA.

EMPLOYEE WAS REPLACING THE REMOVABLE ENDGATE ON TEH FORD F-700 FLAT BED USED TO TRANSPORT EXPLOSIVES INTO THE MINE. A CO-WORKER MOVED THE VEHICLE AHEAD CAUSING THE INJURED EMPLOYEE TO FALL OV ER THE END GATE LANDING ON HIS OUTSTRETCHED HANDS. THE FALL WAS APPROX 4' CAUSING MULTIPLE FRACTURS OF THE LEFT ARM BELOW THE ELBOW.

EE WAS HAULING CRUSHED STONE DOWN A 15 DEGREE SLOPE USING A VOLVO A25 ARTICULATING HAUL TRUCK. EE WAS IN LOW GEAR BUT BEGAN TRAVELING TOO FAST. THE BREAKES WERE NOT ABLE TO SLOW THE VEHICLE AF FECTIVELY THE TRUCK CAB ROLLED OVER ONTO IT SIDE

EMPLOYEE WAS ATTEMPTING TO LOWER A GATE ON A SURGE BIN WHEN THE GATE CLOSED SMASHING EMPLOYEE'S FINGER.

EE WAS CLEANING UP AROUND CRUSHER WHEN HE BUMPEDHIS ARM ON TAHE CORNER OF A SCARP OF EXPANDED METAL RESULTING IN A LACERATION ON THE OUTSIDE OF R FOREARM NEAR THE WRIST. 3 STITCHES TO CLOSE.

EE WAS TRYING TO LOOSEN DRILL STEEL FROM BIT WITH HAMMER. WHEN STRICKING THE STEEL A FRACMENT CUT THE EE S ARM. EE RECEIVED 3 STICHES.

EMPLOYEES WERE LIFTING W/A CRANE A HOUSING TO A PAIR OF JOY AXIAL VAN FANS. THE EMPLOYEE HAD HIS LEFT HAND ONTEH HOUSING WHEN ONE OF THE SUPPORTING CABLES SLIPPED OFF STRIKING THE EMPLOYEE IN THE THUMB.

EMPLOYEE WAS HELPING A GRATE IN THE HAMMER MILL WHEN THE GRATE SLIPPED DOWN CATCHING THE EMPLOYEES HAND.

EMPLOYEE WAS CLIMBING DOWN FROM THE CHUTE AREA OF CONVEYOR WHEN HE CAUGHT HIS RING ON THE CHUTE.THE RING CUT HIS FINGER REQUIRING FIVE STTICHES TO CLOSE.

EMPLOYEE STEPPED DOWN TO FLOOR FROM TRUCK STEPPING ON ROCK AND TWISTED ANKLE.

EMPLOYEE WAS DRIVING A 966 LOADER DOWN THE ENTRANCE SLOPE WHEN THE UNIT BROKE LOAD & FREE WHEELED FOR APPROX 80FT BEFORE WEDGING INTO SIDE WALL OF TUNNEL

EMPLOYEE WAS ASSISTING WITH USE OF AN AIR DRIVEN WRENCH WHEN THE AIRLINE SEPERATED FROM THE TOOL CAUSING THE EMPLOYEE TO LOOSE HIS BALANCE AND FALL FROM THE HOOD OF A CAT 960 LOADER. EMPLOYEE LANDED ON ROCK FLOOR ON HANDS AND KNEE'S.

EMPLOYEE SAW SHAFT ON TELESCOPING BOOM WAS DOWN BELOW GUIDEHOLE ON STOP AND GRABBED THE SHAFT WITH BOTH HANDS.WHEN HE GRABBED SHAFT THE WEIGHT SHIFT IN BASKET CAUSED STOP TO DROP DOWN SHAFT CR USHING EMPLOYEES HANDS.BASKET HAD TO BE RAISED SLIGHTLY TO FACILIATE REMOVAL OF HANDS. Y

MAN WAS WELDING ON TRUCK WHEN PIECE OF STEEL FLEW BEHIND SAFETY GLASSES INTO EYE.

HE WAS CLEANING OUT IMPACT BREAKER WHEN A ROCK ROLLED DOWN AND ROLLED ON HIS ANKLE BREAKING A BONE IN IT.

THE MAN WAS CHECKING RADIATOR AND THE ENGINE WAS STILL HOT. WHEN HE REMOVED THE CAP, THE STEAM AND WATER BURNED HIS LEFT ARM BETWEEN THE ELBOW AND HAND.

THIS MAN WAS NOT ONE OF OUR EE. HE WAS A CONTRACT CARRIER, HAULING MATERIALS FROM OUR QUARRY SIRE. HE WAS EE BY VANDUSS3L SAND AND GRAVE. HE WAS WALKING ON THE TONGUE OF HIS PUP AND SLIPPED OF F AND CAUGHT HIS LEG IN BETWEEN THE BRACE AND TONGUE BREAKING HIS LEFT LEG.

EMPL WAS ACTING FOREMAN WHEN ACCIDENT HAPPENED.DIDNT REPORT IT UNTIO 3-12 TO SUPV.HE WAS CLIMBING DOWN LADDER OF LOADER AND MISSED ONE STEP.DIDNT HIT GROUND VERY HARD AND DIDNT THINK ANYTHING ABOUT IT FOR A COUPLE OF WEEKS -STARTED HURTING.

THE L-140 LOADER OPERATED BY I,NAME , WAS SETTING STILL, WITH THE BUCKET ON THE GROUND. I,NAME WAS SLIGING A SHEET OF STEEL INTO THE BUCKET. AFTER HE DID THAT, HE WAS STANDING ON THE SHEET OF STEEL WITH HIS HAND HOLDING ONTO THE BACK OF THE BUCKET, 2ND MOTIONED TO THE OPERATOR TO TILT HE BUCKET BACK, WHEN HE DID HIS HAND WAS CAUGHT BETWEEN THE BUCKET 2ND AND THE CYLINDER, BREAKING

EMPL POUNDING ON HAMMER WITH SLEDGE PIECE OF STEEL BROKE OFF AND LODGED IN LEFT ARM MUSCLE.

PROBABLE FLAW I3 ROOF SEAM CAUSIED PORTION OF MINE ROOF TO FALL.EMPL OPERATING ENDLOADER WAS HIT BY SOME DEBRIS.
