Employee was attempting to climb a ladder to access a drain plug on a sand sluice pipe. While climbing the ladder the bottom kicked out causing the employee to fall approximately 5' to the ground, bruising left shoulder and left side rib area. Did not wait for a spotter to secure the bottom of the ladder.

Employee was helping to lift a short conveyor frame when ee felt a slight tug in groin. A follow up exam at a medical facility reveled ee has a hernia.

The miner was working on reversing the toggle plates in an Austin-Western jaw crusher. Miner was in the rock box removing a cheek plate when the stationary toggle slid approximately ten inches, and during its travel, it came in contact with the miner's hard hat. As a result, the miner received sprained muscles in back and neck.

EMPLOYEE WAS IMMEDIATLY TAKEN TO THE E MERGENCY ROOM. X RAYS WERE TAKEN. THEY WERE NEGATIVE HIS FINGERS WERE BANDAGED AND HE WAS TOLD TO STAY HOME FOR A FEW DAYS HE WAS ALSO GIVEN A TETENUS SHOT.
