Employee stated EE was loading bolt supplies in the scoop bucket at the supply hole when EE slipped on oil that was in the scoop bucket and fell on EE's right foot. EE was treated at Raleigh General Hospital where EE was treated for a sprained right ankle.

Employee stated that employee was stepping down from the walkway of the bolter to the ground at the back of the machine when employee stepped on a rock and rolled ankle. Employee was treated for a fractured left ankle and released to return to full duty on 3/11/2024, employee's next regular scheduled shift.

Employee stated that they and a co-worker were removing supplies from a rail mounted flat car and had removed one pallet from the flat car with a chain. When the employee tried to step back on to the flat car they slipped and their leg was now on the ground between the scoop bucket and side of the flat car. at this time the scoop rolled forward pinching their left calf.

Employee stated EE was walking back to EE's scoop when EE slipped and fell landing on EE's right hip.

Employee stated had walked up to check on the drill crew when a chain hoist struck EE in groin area. The drillers were attempting to tighten pipe with 2 pipe wrenches and a chain hoist hooked into the wrenches. Pipe wrench broke causing the chain hoist to strike the employee.

Employee stated had been dropped off to help make a belt splice. Employee stated put a new blade in the knife, and was on knees squaring the belt with a utility knife when the knife came out of the belt and struck EE's right leg just above the knee, causing a 2-3 inch laceration.

Employee stated when EE pulled a rock with a slate bar the rock fell and hit EE in the right forearm and hand.

Employee stated as they were hooking up a pull rope on the boom of the miner a rock fell out between bolts landing on their foot smashing the metatarsal down on the foot, sustaining a crushing injury to toes.

Employee stated EE was putting EE's rubber tire ride on charge on the surface and was walking around the ride to get EE's tools when EE's right knee gave out and buckled causing EE to drop to the ground.

Employee stated that they were helping an electrician fix a ride and they and another employee lifted the ride up. When they let it down employee got left thumb caught between the ride and a bottle jack resulting in 3 stitches.

Employee was moving op shuttle car anchor. When they got out of shuttle car they slipped on a ledge hurting right knee

HAD POWER TROUBLE, EVENING SHIFT FOREMAN CAME OUT TO CHECK PUMPS AND DISCOVERED ROOF FALL AT SPAD# 1969 (168 BREAK ON 7 BELT APPROX. 19 FOOT WIDE AND 10 FOOT HIGH AND 50 FEET LONG ON TOP OF HIGH VOLTAGE CABLE).

The move crew loaded belt crossover on mantrip canopy. Employee sat on mantrip batteries while someone else trammed mantrip. Crossover got caught on lifeline and pinned employee's head against canopy resulting in laceration to head

Employee stated that EE was in the process of removing coal that had built up on the side of EE's shuttle car to help EE's visibility. EE stated that when EE pushed the coal into the shuttle car EE's right forearm was cut on a sharp edge of the sideboard caused by wear and tear.

Roof fall 140' long 241 crosscut 8-10 foot high, from #1 to #3 intersection.

WET, MUDDY, UNEVEN BOTTOM. DRAGGING CURTAIN, FELL GRABBED A RIB BOLT TO CATCH SELF AND SEPARATED BICEP.

Employee Stated as the owl crew were departing the motor barn on #1 rail runner mantrip. The mantrip struck the derail and wrecked. The derail was located one foot outside of the motor barn. Injured had a hand positioned at the top of the batteries. When the mantrip wrecked the batteries slid forward catching right pinky finger between the batteries and frame

EE stated that EE was loading a belt cradle into a mantrip to take to 52 Bk. As EE slid the cradle into the mantrip, EE felt a sharp pain in middle/lower back.

Injured stated EE had drilled a 3 foot starter hole and was changing steel to a 3 foot finisher when a rock fell from the roof pinching EE's left ring finger against the drill steel.

A roof fall was discovered approximately 9:00 A.M. on 6/20/2019 2 brks inby spad 2472, 1 brk outby DPP 9. The area was danger and breaker off.

Injured employee stated EE was operating the left Fletcher bolter on the #1 section, stated that EE was installing 10ft cable bolts in the intersection between entries 3 & 4 at #251 break, stated EE was 6ft in depth for a 10ft deep hole, stated that when bolt partner dropped pot it caused stress on EE's steel causing it to break and therefore strike EE's left pinky finger.

Employee stated was walking around a forklift charger and stepped on a rock and lost footing causing EE to twist knee. On 11/1/2018 Employee sought medical care and was placed off work under doctors care.

Employee stated ee and another employee were attempting to place a strap on the T-bar when ee was cut by the strap on wrist during the exchange. The employee received 10 stitches and was placed off work under doctor's orders.

Employee was loading tray on bolter when EE struck EE's right elbow against the bolter resulting lost work days.

Employee was using a chain ratchet when the hook broke and caused the chain to hit employee in the right elbow resulting in a laceration which required 3 sutures.

Employee picked up a wooden pallet and twisted with feet planted and felt a pop in the left knee which resulted in a sprain/strain injury and lost work days.

Employee stated EE was holding a hammer punch while another employee was using the hammer to strike. The employee swinging the hammer missed hitting the frame on a shuttle car and the hammer bounced back striking the other employee in the left forearm

Employee stated EE was tramming the bolter and hit a hole causing EE to be thrown into the canopy striking EE's head against the canopy. Employee was taken off work until 1/12/2018

Employee was struck by a rock resulting in shoulder/lower back sprain/strain and restricted work days.

Employee stated employee was installing roof bolts in the #4 Entry and as employee walked toward the back of the roof bolter a rock fell from between bolts and struck employee on the back of the head/neck/shoulders. The rock measured approximately 4'L x 2'W x 0" to 2"T. This resulted in a strain and restricted work days.

Employee stated was working in the 3 Left Crosscut, installing the first roof bolt of the day. As EE pushed the bolt into the hole, EE felt a burning sensation in the left shoulder. This resulted in a shoulder strain.

Employee stated that ee was operating the #15 mantrip at 94XC when the seat came loose and fell off of the ride. EE struck left hand and left knee, resulting in strains and lost work days.

Employee stated was operating a scoop and encountered a communication and junction box hanging from the mine roof. Employee used left hand to hold the cable away from the canopy while operating the scoop. Employee's L index, middle, and ring fingers were caught between the canopy of the scoop and the rib, resulting in an amputation and lost work days.

Employee stated ee was bolting in the #4 entry on the #1 section when a rock fell on the mast. The employee tossed the rock off of the miner straining right shoulder. This incident resulted in restricted work activity.

Employee stated EE was standing inside the bucket of the scoop and loading supplies onto the roof bolter. Emptied a wooden pallet and turned to throw it out of the scoop bucket. As EE turned EE's feet slipped and EE fell over the side of the scoop bucket resulting in sprains to multiple areas of EE's body and restricted work days.

Employee stated was removing shields off the pump motor compartment of the scoop and felt a burning sensation down neck and shoulders.

Employee bent over to throw the track switch located on the surface of the mine yard. Employee states foot slipped in mud, causing pain in lower back. The incident became reportable on 9/30/2016 when the employee began missing work.

An unintentional roof fall occurred in the #1 Entry (Intake) at Break #25. The fall was approximately 120' long x 19' wide x 10' thick.

Employee stated was changing scoop batteries. As EE turned the battery bell, a metal tag with a sharp edge cut left index finger. This resulted in a laceration, and lost work days. Sutures were used to repair the wound.

Employee stated that they were operating the scoop along the roadway when the scoop hit a hole. This caused the employee to be jarred in the operator's compartment. The incident resulted in a back strain and lost work days.

Employee was walking on the mine yard when he became ill and collapsed to the ground in an unconscious state. He was transported to Raleigh General Hospital where he was pronounced deceased. The actual cause of death is unknown at this time. The M.S.H.A. was notified and an investigation was conducted by the agency.

Employee stated he was loading the roof bolt machine with supplies when he felt a burning sensation in his abdomen. Employee was diagnosed with a hernia.

Employee stated that when he bent over to pick up a bundle of plates he felt a pain in his lower back.

Employee stated that he was struck in the right leg by a stack of pie pans that had fallen off the top of the canopy of the Roof Bolter Machine.

Employee stated that he was using a slate bar to pry a plate that was caught in a pivot point on the bolt machine when the bar slipped causing him to strike his hand on the frame of the bolt machine. This broke the nail and bone on the tip of his right index finger.

Employee stated that he was attempting to hang the line curtain on a curtain hook where the top was too high for him to reach from the ground. The employee jumped up to snag the curtain on the hook, but cut his right index finger in the process requiring 6 sutures.

Employee stated that while he was attempting to place a roofbolt into the drill pot, the plate became lodged in the drill steel guide. When the employee attempted to remove the fouled plate, his right thumb was pinched between the plate and the drill guide, resulting in a fracture.

Contractor was loading truck after work at mine site was finished and stepped off tail gate and slipped on mud and twisted ankle. It was his right ankle and he was unable to drive to ER therefore an ambulance was called.

Contract labor emplyee stated that as he was walking in front of the bolt machine being trammed to break 43, he had stopped to observe a peice of draw rock when the bolt machine tire came in to contact with his left foot. The contract laborer was walking on the off side of the machine when the incident occurred.

Employee stated that as he was dismounting the bobcat, he stepped in a hole and twisted his left ankle. Employee went to the doctor on Monday 1/19/2015 where he was placed on restricted duty and his classification was changed to accommodate his restrictions.

A continuation of a previously reported roof fall on the First Northwest mains panel, which is a worked out area. The fall continued across the #1 entry at break #84. This condition restricts passage in this area.

Roof Fall in the #2 entry inby spad #688 approximately 30 feet from face. The fall is estimated to be 200'L x 19'W x 15'H.

A continuation of a previous roof fall located in the Northwest Mains, the fall extending from the #5 entry to #2 entry, and is approximately 240'L x 20'W x 20'H.

Roof fall dated 6/3/2014 was located in the #6 entry of the mains in by spad #530. The fall was approximately 40'L x 18'W x 10'H.

Roof fall dated 6/2/2014 was located in the #9 entry of the Northwest main 1 break out-by the face. The fall was approximately 40'L x 18'W x 10'H.

Employee was walking toward the outside in the secondary escapeway, and slipped injuring his left calf.

Roof fall dated 5/12/2014 was located inby the feeder in the belt entry. The fall was approximately 30'L x 18'W x 10'H.

IE was roof bolting, started to insert his pusher steel when a rock fell and struck him on the left forearm causing a laceration.

Employee was operating the #2 Fletcher Roof Bolter in the #4 entry of MMU, 001-0. As employee was drilling his wrench kicked out striking his little finger on his left hand causing a dislocation.

Employee was operating the scoop in the #9 entry. As he removed himself from the scoop, to retrieve some rock dust he slipped, and landed on his left hand, causing a sprain to his wrist.

Employee was inserting roof bolt into hole. The bolt bent, and the ring finger on his left hand was smashed against the mast on the Roof Bolter which, resulted in employee receiving two stitches in his finger.

EE was building a intake stopping, as he placing the block, the ladder started to shift causing ee to fall. As ee was falling the block fell striking him on his left collar bone causing a fracture.

A piece, of rock fell across the miner. While, ee was removing the rock with a slate bar, the rock broke in half, and the slate bar slipped striking ee in the nose causing a laceration. After finishing the shift ee went to the Doctor, and received butterfly stitches.

Employee was standing on a ladder installing fly pads in the #2 entry. While hanging the fly pads, the scoop operator trammed through pads striking the ladder, causing him to fall.
