Was using 745 Hi-Lift to clean mud off haul road, caught bucket on edge of bridge. Driver hit head in cab, was taken to E.R. & got 4 staples. He was back to work that day.

OPERATOR WAS EXITING FRONT ENDLOADER PROPERLY AND SLIPPED AND FELL OFF STEPS TO THE GROUND. SLIP WAS CAUSED BY OIL ON STEPS FROM FILLING HYD. TANK & OPERATOR WAS WEARING BOOTS WITH SMOOTH SOLES.
