A nerve test was conducted on July 14th that identified moderate carpal tunnel in both wrists. No direct cause was identified. There is no accident date and time for this condition as it wasn't connected to a single incident.

After the shift at New Harvey EE was alone in the scale house. The leadman had already left for the day. EE had chest pain strong enough that he called his family to come and get him. EE was taken to the hospital, they confirmed he had multiple heart attacks.

EE REPORTED PROBLEMS WITH HER HAND DURING A LAYOFF PERIOD. OVER NEXT COUPLE OF MONTHS EE WAS SEEN BY A DOCTOR AND SURGEON. EE WAS GIVEN FUNCTIONAL NERVE TEST TO DETERMINE IF SHE HAD CARPAL TUNNEL SYNDROME. EE WAS DIAGNOSED WITH CTS AND SURGERY WAS PERFORMED ON 5-14-01. EE RETURNED TO WORK AT REGULAR CAPACITY ON 5-17-01.

EE WAS DOING WELDING REPAIRS ON SAND CLASSIFIER AND STEPPED ON A 4"X4" SECTION OF WOOD THAT WAS BEING USED DURING THE REPAIRS. THE 4"X4" ROLLED OVER CAUSING THE EE S LEFT ANKLE TO FRACTURE.

EE WAS USING CUTTING TORCH INSIDE OF A DIAL SPLIT CLASSIFER. 5 GALLON PLASTIC BUCKET WAS ABOVE EE ON A CATWALK. WIND BLEW BUCKET OFF OF CATWALK AND STRUCK EE IN THE HEAD RESULTING IN 5 STAPLES .
