On March 22, 2016 at approximately 6:35pm an employee was struck by flyrock after a shot was detonated. The employee was sitting inside of a pickup approximately 1,200 feet away from the shot when detonated. This incident resulted in a fatality.

EE bent over to pick up a bottle of hydraulic oil and strained back.

Employee was trying to move a track off of a CAT D8R Dozer to perform repairs. The tension got tight and the chain slipped off causing the hook on the end to come down and hit the employee in the chin and cutting it. Employee received 4 stitches.

Technician was mounting an oil cooler assembly onto a CAT D400 truck. Had to use a pry bar to help get the cooler mounting bolts lined up with the bolt holes. The pry bar slipped off of the cooler, forcing the tech's left hand to hit the head of another bolt on the machine, lacerating the top of the knuckle on his middle finger. Also, the rain made all of the equipment wet.

EE cut his left wrist with a utility knife while attempting to cut a patch for a conveyor belt. EE was pushing the knife away from his body but contacted his left wrist when the knife slipped. Resulting laceration severed 3 tendons that required minor surgery to repair.
While replacing steering hose on D8R.He was down under floor plates reaching in a bad spot to change a hose, must have over reached pulling muscle. It didn't feel bad at the time so didn't tell supervisor for two days. On Friday arm felt bad and fingers a little numb, went to chriopractor.

EE was dismounting loader at designated parking area and stepped on side of berm causing him to sprain his ankle. Did not report incident to manager until 06/20/2008.

Employee got his finger between slide bar and spinner pinching his index finger of his left hand. EE thought that he had only damaged his fingernail. A band aid was placed on it and the EE continued to work. EE went to the Doctor on Oct. 26 and was diagnosed with a fracture at the tip of his index finger. No treatment was needed.
