EE bent over to pick up a bottle of hydraulic oil and strained back.

Employee was trying to move a track off of a CAT D8R Dozer to perform repairs. The tension got tight and the chain slipped off causing the hook on the end to come down and hit the employee in the chin and cutting it. Employee received 4 stitches.

Technician was mounting an oil cooler assembly onto a CAT D400 truck. Had to use a pry bar to help get the cooler mounting bolts lined up with the bolt holes. The pry bar slipped off of the cooler, forcing the tech's left hand to hit the head of another bolt on the machine, lacerating the top of the knuckle on his middle finger. Also, the rain made all of the equipment wet.

EE cut his left wrist with a utility knife while attempting to cut a patch for a conveyor belt. EE was pushing the knife away from his body but contacted his left wrist when the knife slipped. Resulting laceration severed 3 tendons that required minor surgery to repair.

While replacing steering hose on D8R.He was down under floor plates reaching in a bad spot to change a hose, must have over reached pulling muscle. It didn't feel bad at the time so didn't tell supervisor for two days. On Friday arm felt bad and fingers a little numb, went to chriopractor.

EE was dismounting loader at designated parking area and stepped on side of berm causing him to sprain his ankle. Did not report incident to manager until 06/20/2008.

Employee got his finger between slide bar and spinner pinching his index finger of his left hand. EE thought that he had only damaged his fingernail. A band aid was placed on it and the EE continued to work. EE went to the Doctor on Oct. 26 and was diagnosed with a fracture at the tip of his index finger. No treatment was needed.

EMPLOYEE WAS USING SAFTEY GLASSES & A FULL FACE SHEILD AT THE TIME. (METAL FRAGMENT IN EYE).

EE WAS CUTTING A PIECE OF RUBBER FLASHING WIPER OF THE ROAD STONE BIN WHEN THE UTILITY KNIFE SLIPPED CUTTING THE EE'S LEFT FOREARM.

EE WAS LIFTING STANDING PLATFORM ON DRILL. THE PLATFORM HAD MUD ON IT WHICH CAUSED THE EE TO STRAIN HIS LOWER BACK.

WHILE CUTTING BRUSH WITH A CHAIN SAW, THE SAW CAUGHT ON A SMALL TREE AND KICKED BACK INTO THE EMPLOYEES LEFT KNEE. THE KNEE REQUIRED 25 SUTURES.

EE RAISED HIS ARMS AND STARTED TO PULL HIMSELF INTO HIS TRUCK WHEN HE FELT SOMETHING IN HIS BACK POP. THIS WAS FOUND TO BE A DISLOCATED RIB.

EMPLOYEE STATED THAT HE GOT SOME LIMESTONE DUST IN HIS EYES WHILE OPERATING A DOZER HE EVIDENTLY RUBBED HIS EYES CAUSING THE CORNEA OF THE RT EYE TO BECOME SCRATCHED HE MISSED THURSDAY MARCH 1 8 TO ALLOW HEALING TO OCCUR

ELMPLOYEE STATED HE WAS ATTEMPTING TO REMOVE A ROCK JAMMING THE IMPACT. THE ROCK SLID, PINNING HIS HAND AGAISNT A LINER IN THE IMPACT. THE INJURED RESULTED TO STITCHES TO HIS LEFT INDEX FING ER.

EMPLOYEE WAS ST5NDING ON TIRES REMOVING BOLTS FROM FENDER ON PRIMARY HE STEPPED FROM ONE SET OF T IRES TO ANOTHER SET OF TIRES SECOND SET WAS ON AN AIR AXLE THIS SET WAS OFF GROUND & ROLLED CA USING EMPLOYEE 7O FALL TO GROUND RESULTING IN A BRFEAK T O HIS RT.FOREARM

EMP WAS TIGHTEN3NG BOLTS ON IMPACTOR V-BELTS GUARD, 1/2" RATCHET SLIPPED OFF OF BOLT STRIKING HIM IN MOUTH. BROKE HALF OF RIGHT UPPER FRONT TOOTH AND CHIPPED LEFT UPPER FRONT TOOTH
