DRIVING TRUCK UP THE INCLINE, LOST CONTROL AND TRUCK WENT BACKWARDS AND FLIPPED ONTO IT'S TOP. THE DRIVER SAID HE WAS NOT HURT. TOOK HIM TO LOGAN PRIORITY CARE TO BE CHECKED OUT. THE DRIVER RECEIVED A SOFT TISSUE CONCUSSION.

MY LEGS WERE RESTING BETWEEN TWO PIECES OF METAL WHILE WORKING ON A BEARING. THE METAL WAS WARM, BUT NOT HOT TO THE TOUCH. BOTH LEGS ARE BURNED ON EACH CALF. THE BURNS ARE ABOUT THE SIZE OF A BASEBALL. I DID NOT NOTICE THE BURNS UNTIL I GOT HOME LATER THAT NIGHT. I REPORTED THE ACCIDENT TO MY SUPERVISOR THE NEXT MORNING.

OPERATOR STARTED UP RAMP WITH WATER TRUCK. THE VEHICLE STARTED TO SPIN, HIT DIFFERENTIAL LOCK, BUT DID NOT HELP. TRIED TO ROLL BACK DOWM INCLINE, LOST CONTROL OF VEHICLE, CAUSING IT TO ROLL OVER DOWN INCLINE

The operator was shoveling under a belt line, the man claimed his back was sore and painful, he went to a doctor after the shift and told us about it the next day. He did not report to the supervisor at 5:00 PM same day. We are not sure he is hurt or faking it.
