FROM PRELIMINARY: THE SCRAPER WAS ASCENDING A RAMP AFTER BEING LOADED AND STOPPED JUST AS IT APPROACHED THE CREST. IT TRAVELED BACKWARD DOWN RAMP ABOUT 90 FEET, PROCEEDED OVER THE HIGHWALL AND DROPPED 78 FEET TO THE QUARRY FLOOR.

CONDUCTING CONTINUITY AND RESISTANCE OF GROUND SYSTEM. A FALT OCCURRED IN THE ENERGIZED 480 VOLT MAIN DISCONNECT SWITCH FOR OIL HOUSE RESULTING IN PHASE TO GROUND AND PHASE TO PHASE FALT. THE HIGH TEMPERATURE ARC CAUSED BURNS TO FACE AND HANDS AND ELECTRICAL SHOCK.
