EE WAS STANDING ON CATWALK HOLDING ONTO ROPE THAT WE USE TO PUT HOIST CABLE ON TO KEEP FROM HAVING TO WALK THE BOOM, WHEN THE ROPE WAS PULLED BY ANOTHER EMPLOYEE. THIS TWISTED EE AROUND AND PULLED A MUSCLE IN EE'S BACK.

EE WAS TRYING TO UNHOOK A CABLE TRAILER FROM HIS PICKUP. HE WAS JUMPING UP AND DOWN ON THE BUMPER OF HIS TRUCK WHEN IT CAME UNDONE FROM THE PICKUP, STRIKING HIM AND TOSSING HIM INTO THE AIR (APPROX 4 FT) AND OVER THE TRAILER. HE LANDED ON HIS BACK AND SHOULDER ON THE GROUND.

EMPLOYEE WENT DOWN IN PIT TO START PUMP, FOUND SUCTION LINE BURIED IN MUD, TRIED TO PULL OUT ON ROPE, WHEN HE FELT PAIN IN HIS LOWER PART OF HIS BODY. HE WENT TO DOCTOR ON 7/14 WHO SAID HE HAD A HERNIA AND NEEDED CORRECTIVE SURGERY.

EE WAS OPERATING A KOMATSU EXCAVATOR. THE TEMPERATURE GUAGE WAS SHOWING HOT. HE STOPPED TO CHECK WATER IN RADIATOR. HE DID NOT LET THE UNIT COOL DOWN BEFORE REMOVING THE RADIATOR CAP. THE HOT WATER PRESSURE BLEW OUT ON HIS RIGHT ARM & STOMACH.
