This employee started missing time on 3/15/11. On 3/15/11 this company received notice that employee filed with KY office of workers claims. This was reported to out compension currier on 5/5/11

One of the trucks over traveled the berm causing the bed to over turn. The employee was thrown around in the cab causing alleged injury.
