BACKHOE OPERATOR WAS DIGGING MATERIAL OUT OF A WATER HOLE. HAD TRACKS PARALLEL TO DIGOUT. WALL CAVED IN AND THE BACKHOE LEANED INTO THE DIGOUT.
EMPLOYEE WAS SERVICING (FUELING) THE DOZER WHILE STANDING ON THE CATWALK HE SLIPPED DUE TO THE MUD ON HIS SHOES & FELL HITTING THE LATCH OF THE FUEL TANK ON HIS WAY TO THE GROUND
