EMP WAS WALKING DOWN THE ENTRY AND HIT HIS BACK ON ROOF BOLTER DID NOT CEASE WORK REPORT NOT FILED BY EMP FOR 2 WEEKS NO TREATMENT MEDICATION ONLY
HAD PAIN IN LOWER BACK WHILE DRIVING SHUTTLE CAR.NO USUAL INJURY OR CAUSE COULD BE DETERMINED.
BRUISED LEFT SHOULDER WHILE LIFTING A HEADER.
EMP BUMPED HIS HEAD ON HALF HEADER WHILE WALKING
A TRAILING CABLE CAME LOOSE KNOCKING THE EMPLOYEE DOWN CAUSING A PULLED NECK MUSCLE AND BRUISED KNEE. EMPLOYEE WAS EXAMINED BY DR. AND SENT TO SPECIALIST FOR FOLLOW UP AND WAS THERE RELEASED T O RETURN. HE RECEIVED NO MEDICAL TREATMENT OTHER THAN BEING ADVISED NOT TO OVEREXERT.
