THE EMPLOYEE OPENED THE VALVE TO DRAIN THE WATER OUT OF THE AIR TANK ON THE LUBE TRUCK, NOTHING CAME OUT OF THE HOSE. HE ASSUMED NO AIR BUILT UP IN THE TANK. HE TOOK ONE STEP TOWARD THE CAB TO START THE TRUCK AND BUILD UP AIR. AT THAT TIME THE DRAIN HOSE BROKE LOOSE AND STARTED WHIPPING.THE FORCE HIT HIM IN HIS RIGHT EYE, CHEEK AND FOREHEAD.(THING IN HOSE END MUST HAVE BLOCKED AIR)

AT 12:00 EMP WAS GETTING OFF OF HIS LOADER. WHENHE STEPPED DOWN FROM THE TIRE TO THE COUNTER WEIGHT HIS FOOT SLIPPED AND HE FELL DOWN THE SIDE OF THE STEP. A KNOT APPEARED ON HIS RIGHT SHIN AN D HE SCRAPED THE SIDE OF HIS LEG.
