Blackjewel L.L.C. · Struck by falling object

Employee was placing filter fabric over drain, climbing layers of rock. While climbing, employee rested hand on rock and it moved. Employee fell with rock striking right lower leg and foot. ER exam showed fractured fibula 3in. above ankle.

Blackjewel L.L.C. · Over-exertion in lifting objects

Employee was welding on mixing table box, finished welding one end, EE stood box up in floor to finish welding other side. Box weigh approx. 150 lbs. When standing box up EE felt burning sensation in stomach area near belly button & felt small knot. EE has hernia & had surgery on 04/04/2017. EE is off work until release by doctor.
