Working on #6 Scoop. Employee was pulling cable leads through rubber grommet with channel locks when they slipped loose and channel locks struck above right eye. Employee was taken to ER were EE received 3 stitches.

Installing belt clamps to make belt splice, hammering two wedges between clamp and belt. Top wedge broke causing hammer to deflect and struck injured in right cheek below corner of right eye approximately 1 inch, causing a 1 inch laceration. Taken to a medical center requiring 2 stitches to close laceration.

Changing wheel bearing on 3-wheel ride (working alone), 3-wheel was on jack stands, ee may have bumped vehicle causing it to tilt over & fall on legs. EE went to ER, no fractures or contusions, on crutches and off work to follow up with orthopedic doctor.

EE was backing ride up & EE's hard hat struck a roof bolt. EE had neck pain immediately. EE continued to work 03/21/2017 thru 03/24/2017. On the 24th EE worked a half day, EE left early due to pain & went to local ER. EE was seen & release to return to work on the 28th, EE was told to follow up with family doctor. EE saw family doctor on 28th & has EE off until further notice.

Employee positioned roof boting machine, set ATRS sounded top and began drilling to install a bolt. A piece of roof rock 5"x5"x2'7" fell from roof, hit ATRS & struck ee on left side of head & neck.

EE was traveling back outside from a remote area of the mine. In location inby Spad #1551 a roof rock 12x16x3 fell striking EE on left side of face & shoulder. Impact of rock caused a 1" cut on side of face did not require stitches. EE lost consciousness for a period & regained consciousness and called for assistance.

The injured ee was re-arranging miner cable on the mine floor when he fell backwards onto his back. The ee continued to work; a few minutes later he stated he felt back pain. He was treated on the section and transported to the surface where an ambulance transported him to the local hospital.

EE had finished installing roof bolts and was advancing the line curtain to provide ventilation to the face. He raised himself up off his knees and felt a tightness in his left knee. EE was administered first aid and transported to the surface where he was taken to the local hospital.

A utility man was injured when the rubber tired vehicle he was riding was struck by a second vehicle. The ee (passenger) was exiting the mine at the end of his shift when the second vehicle which had been parked in a crosscut suddenly and unexpectedly backed out and into the path of travel.

The ee was in the process of moving a trash dumpster using an endloader at the Slate Creek Portal. He had dismounted the loader and was walking across the level surface area when he tripped and fell resulting in a fracture to his right ankle. He was taken to Clinch Valley Medical Center for treatment.

The SC operator was making a right turn into the #3 heading when ee over steered SC & struck a row of timbers. The ee alleged that he moved over & raised up in his seat to avoid being hit by the timbers & struck his head on the mine roof. The ee had 12" of clearance over his hard hat. This report is being submitted under protest. The ee resigned on 5/7/12.(attachment to follow)

A shuttle car operator was installing a ventilation curtain in the #5 crosscut when her foot became stuck in the mud causing her to trip and twist her right knee. The ee reported the incident to her foreman & continued her regular duties. The EE continued to work her regular shift-until 5-2-12 when she went to the doctor & he told her to take off from work for the next 3 days.

A scoop op.was traveling in the #3 entry. The life line for the secondary escape way was on the right side of the entry over the scoop op's compartment. The life line became hooked on the deck of the scoop and as the operator made a right turn the life line and spool became tangled and entered the operator's compartment and struck the op. in the upper lip resulting in a laceration.

The 2nd shift production crew was exiting the mine on a Mac 8 rubber tired mantrip near break 18 when a slate bar that was located on the mantrip caught the mine floor resulting in the bar being impaled into the upper leg of an ee that was seated on the left side of the man trip. The ee was taken to the hospital were surgery was performed to remove the slate bar.

(D-30A) A ROOF FALL was discovered in the #5 entry at survey station 63-68. The fall area was approximately 120' in length, entry wide and 7-8' in thickness. The area was bolted with 42" conventional type bolts. Timbers were set to prevent access and Heintzman roof support jacks were set to protect the #4 entry.

A ROOF FAll was discovered in a remote drift opening in the #2 entry 20 feet outby survey station 671 approximately 25 feet from the surface. The roof fall measured 20 feet long X 18 feet wide.

A ROCK FALL was discovered two breaks inby survey station #1770 in the #5 entry of the 13th development panel during the weekly exam of the outby area. The immediate mine roof at the fall location was slate and the fall measured 30 feet long and 18 feet wide.

The employee was installing roof bolts in the #3 right crosscut (3rd bolt in the 3rd row) when a kettle bottom measuring 70" X 57" X 0 to 8.5" fell & struck him on the left side & trapped his left leg under the rock. A co-worker removed the rock from his leg. First aid was given and emp. was transported to the hospital.

The employee was riding the mantrip with the section crew at the end of his shift. As the mantrip traveled through a ventilation curtain, the miner trainee raised his left leg to hold up the curtain. The employee's leg was caught on the curtain board resulting in a hyperextension of his knee.

Employee was tramming a single head roof bolter & was operating the tram levers with his right hand and his left hand was positioned inside an empty canopy post. An extra section of drill steel had been inserted into the canopy post that extended 18 inches above the machine. The drill steel contacted the mine roof and the operator's middle finger was caught between the steel & post

The security guard was performing a walk around patrol when a patch of snow covered ice collapsed under her foot. This created a hole 6 inches deep and 12 inches wide resulting in her falling and fracturing her right wrist.

The employee was working as a belt drive person performing routine clean-up around the #6 drive area. The employee moved a metal plate (24" X 36") from the drive area and was in the process of positioning the plate against the coal rib when his little finger on his right hand was caught-between the metal plate and the rib.

In process of removing 14-10 miner head from miner, employees were working on ripper chain to remove chain pins. They used a slate bar and come-alongs to loosen chain. After pins were pulled with slate bar in chain the back part of chain was fouled against the slate bar. Miner man let head down to tram forward, when slate bar shot out of chain hitting employee.

THE FALL WAS LOCATED BETWEEN NO. 4 AND NO. 5 HEADING, 3000' FROM FACE IN THE CROSSCUT RIGHT OF S.S. NO. 451.
