Employee stated as EE was bolting in 3 left crosscut a rock fell from the top hitting EE in the back.

As the airway examiner was examine the weekly intake airway a small 10'x10' roof fall that pulled 4 roof bolts was found just inby the fan house, the lifeline was rerouted to another entry, we submitted a plan to clean and rebolt the area that has been completed.

Rockwell Mining LLC · Struck by falling object

Employee stated EE was installing roof bolts in the #2 entry when a piece of draw rock fell, striking EE on the right side of EE's head. This caused a laceration to EE's right cheek that required sutures. Employee returned to work on EE's next shift with a full duty release.

Employee stated that EE had backed the miner out to set over in the cut when a section of the rib rolled out and struck EE on the right hip/back and leg. Employee was treated and released by a doctor and continued to work until 2/24/2025 when EE went back to EE's doctor and was taken off work.

Employee stated that EE and another employee were installing belt hangers in the belt entry on the section. When EE was pushing the bolt up into the top, the 2-foot drill wrench EE was using broke and a piece of it struck EE in the mouth causing a laceration to EE's upper lip. The employee received 3 sutures in EE's lip and returned to work on EE's next scheduled shift.

Employee was notified of an occupational illness and was awarded 10% partial disability as a result. Employee was diagnosed with occupational pneumoconiosis.

Employee stated that EE was carrying a stopping block from the scoop bucket to where they were building a stopping when EE's feet got caught up in the bands from the block. This caused EE to trip and fall where EE struck EE's right hand on the mine floor which resulted in a fracture to EE's hand.

A non-injury roof fall occurred on the #1 Section, in the #8 entry while retreat mining was being performed. The fall occurred halfway between #11 and #12 line of breaks approximately 50' inby SS#: 2056.

Employee stated that while in the 2nd lift on the right block of the #4 entry on the Craney Mains Panel, the employee was attempting to help hang the MRS cable back up when a section of draw rock fell from between the bolts and struck the employee on the head, back and legs which knocked employee to the ground.

EE stated that EE was operating an end drive shuttle car when EE drove through a hole in the mine floor. This caused EE's head to be jarred into the canopy which caused pain in EE's neck. Employee continued working but had to have surgery on 8/29/2024 (this is also the first day of work EE has missed).

Employee stated that EE went to trouble shoot a D box cable and found a section of draw rock had fallen and struck the cable. When EE bent over to get the rock off the cable so repairs could be made, a section of draw rock fell out striking EE on the back of the head. Employee received 9 staples at the ER and returned to work on EE's next scheduled shift.

Employee stated that EE was loading the #1 shuttle car, in the 3rd lift of the #6 entry when a rib rolled out and struck the injured employee on the lower left leg/ankle. This mine was retreat mining at the time the accident occurred.

Employee stated: was trying to remove a stuck cutter motor from the #255 miner and had a chain hooked to the motor from the service scoop. They put pressure on the chain and was hitting the top of the miner with a sledgehammer. The chain broke and the boom of the service scoop hit employee on the left middle finger.

A fall was discovered on 7/2/2019 during the weekly airway examination. The fall was located in the left return at brk 23 #2 entry. The fall occurred in the intersection. The route of travel was not disrupted. Operator built 9 point cribs on all 4 access points and danger off.

A roof fall occurred inby the feeder on 5/11/2019. Break 24 in #1 entry to brk 26 in the #1 entry. Then from brk 26 in 0 entry in brk 29 in 0 entry, all in left return. Area has been cribbed off in all approaches to fall and cable bolted with 12' cable bolts.
