On 5/11/23, operator was using a dolly to move ten-50lb bags of material. Operator pulled dolly back, lost their footing and caused the momentum of the material to push the operator down and they fell on their left knee. The operator was treated with first aid for knee abrasion. Operator did not want to see a doctor when injury occurred. Operator requested to see a doctor 5/19/23.

Employee was torquing track roller bolts on a D8-R dozer when torque wrench internal gears stripped out of ratchet head of wrench. Employee was in squatting position when torque ratchet failed causing employee to lose balance and strike the floor hitting elbow.

Employee was in the process of removing a final drive sprocket from a Komatsu PC600 excavator by striking the sprocket with a metal hammer. He hit the sprocket twice and on the third swing a small piece of metal broke off the sprocket striking and embedding in employee's left inside forearm.

Employee was descending a 6 foot step ladder when his left foot slipped off the bottom rung of the ladder and his right foot came in contact with the floor. EE alleged knee injury was related to this incident. Claim was contested and later accepted under workman's compensation. EE was severed during a plantwide workforce reduction on 5/19/08 and never returned to work.

Two bolts on the air tank were pushing into his belly and eventually caused pain in the employee's groin area. The employee was performing the task for 4 hours off and on.

Employee was removing chains from a compactor when he slipped in the mud. As he was falling, he reached out to steady himself and caught hold of the exhaust pipe of the compactor, sustaining 1st & 2nd degree burns to his right hand.

Attempted to enter the main quarry shop. The shop was locked. Attempted to unlock the door and was unsuccessful. Tried an alternate door. Entered and turned lights on. Before fully illuminated, he entered an unlit walkway. He tripped over a crate and struck a table or chair, causing upper lip lacerations. He was given 3 stitches and released to regular duty and no restrictions.
