Operator was cleaning up material spillage from the Mill White Belt, when they started showing signs of heat illness. Employee was administered fluids.

Employee was loosening a bolt on a brake caliper of a pickup truck, when EE felt a pain in EE's back. EE was diagnosed with a lumbar strain.

The employee was lowering a guard in to position and the guard got hung up while it was being lowered. The employee had hand positioned on the bottom section of the guard and when the guard freed up, it pinched the employee's right index finger, causing a hairline fracture to the finger.

While sliding telehandler forks inward to accommodate a narrower pallet employee pinched finger in between fork and mast cutting the inside of the pinky finger on right hand.

Employee began feeling ill and told a co-worker that EE thought EE may be having a stroke. Diagnosis was a personal health issue of a complex migraine that causes stroke-like symptoms.

Employee was servicing equipment in Harley Pit and began feeling exhausted and dizzy which led to momentary loss of consciousness - presumably related to heat.

Employee was collecting a transmission oil sample on a front-end loader. The sample catch tube was attached to the pressure test port and when it opened the valve, transmission oil was sprayed into the employee's eyes, requiring them to be flushed and eye drops administered.

Employee was walking from MCC back to loader when EE presumably stepped on a rock or uneven surface causing pain on the bottom of EE's left foot and EE's two outside toes.

Employee slipped on the wheel chock berm at ground level while servicing a FEL. The employee landed on EE's lower right back area and experienced a bruise/contusion to the area.

A wildfire that started near Hitchcock (Blaine County 720 Fire) burned onto USG property. When it reached the property, we notified MSHA. It burned several acres on the mine site. No personnel were injured and no equipment was damaged.

Wind gusted around the DEF storage area, blowing airborne dust into employee's left eye. Employee incurred a scratched cornea.

Employee was repositioning a jack-stand, and while maneuvering the stand, it shifted and the employee placed EE's right hand on the top of the stand to prevent it from tilting forward, resulting in pinching EE's right thumb in between the stand and the frame of mobile equipment EE was working on. The tip of the right thumb was fractured.

Employee was washing a pickup truck at the wash pad. Employee was washing from the bed of the pickup and fell over the tailgate to the ground. The fall resulted in a fractured left thumb, rib, and transverse process.

Employee was removing an old light fixture when the fixture slipped from grip and while catching the fixture, a sharp bolt end cut the employee's right ear.

Operator of the EX1200 Excavator was dismounting machine when the operator stepped on a rock on the ground, rolling left ankle. The ankle was determined to be dislocated and fractured.

Employee was operating the surface miner when EE started feeling ill to the stomach. EE notified the supervisor immediately and was treated for heat illness (cooled off and hydrated). Employee was later taken to dr and received IV fluids.

While stepping up from ground to first step on Haul truck, employee felt a pop in EE's knee.

Employee was walking into work from the parking lot. Employee stepped on top of a curb. Upon stepping down on the other side, employee rolled an ankle on the uneven surface.

While assisting the blaster load a shot, employee moved a shovel and while holding it to move, employee twisted at the waist and felt pain in EE's back.

During the day employee was helping to cut and empty reclaim bags in the pit so it could be mixed in with the shot. Employee got something in eye and rubbed the eye. Employee scratched the cornea and was given antibiotic eye drops.

Employee picked up the oil sample from the back of the shop pickup and turned and tripped on engine stand landing in the gravel causing lacerations to EE's face.

Employee was driving 06 Haul Truck down Hokie Pit Road, where other equipment had been stuck in the mud on the previous day. There was ruts in road where dozer had turned to pull out stuck equipment. Employee hit ruts, causing employee to bounce and seat suspension bottomed out, jarring employee's back. Employee was taken to clinic and released.

Employee was using a cutting torch. EE removed EE's glove to clear some items out of EE's way. EE passed EE's hand between the torch and the work bench causing a 2nd degree burn to the back of EE's hand.

Operator just dumped a load in the dump area from the CAT 657G scraper. As they were leaving the dump area, they drove through an area of roughness as they were lowering the apron. The rough ground jarred the scraper, causing the operator's left shoulder to strike the safety bar on the left side of the operator's cab causing a fracture to left clavicle.

Employee was driving the loader down the haul road and the loader went over a rut causing the loader to bounce & the seat bottomed out.

Employee was removing an elevator chain pin that was wedged between the idler and elevator casing. While pulling the pin, the employee placed right hand over the end of the pin. Another mechanic who was assisting in this job was jogging the elevator. The elevator rolled back wedging the pin against the floor and the employee's right hand.

Employee was helping to set up 6 inch water pump. After loading 6 inch discharge hose with another employee on a roll of hose, EE helped pick up roll and turned to put hose in bed of pickup when EE felt a pain in the lower back.

Employee was disconnecting and rolling up 4" flat discharge hose from a trash pump. As employee attempted to unlatch the QD fitting, the right hand was cut by the end of the band-it clamp tail causing a laceration on base of right thumb. Employee saw doctor and received 2 stitches.

EE was operating the loader, the air conditioner did not work so EE opened up the windows for air. At 2:00 pm left eye was irritated. In the morning when EE woke up the eye was almost swollen shut. EE was taken to doctor (5/27/16) where the doctor did not find anything in the eye. The doctor diagnosed as allergy, gave employee steroid eye drops and released employee to work.

Two employees were removing a drill steel from a storage rack. As they pulled the drill steel towards themselves, the other end came off the racking storage arm. This caused the up swinging end to pinch the injured employee's hand against the underside of the next level racking arm, causing a laceration/avulsion to left hand ring finger base knuckle.

Employee was doing crusher clean up and EE turned on overhead fan. The switch to the fan was located over EE's head and while EE looked up to flip the switch, some material from the fan flew down into eyes.

Quarry mechanic was backing a portable 4" pump into the Quarry Crusher Maintenance shop. When the mechanic released the pump trailer hitch from the pickup bumper ball, the pump trailer moved forward, pinching his right index finger between the trailer hitch and rear bumper. Employee had a laceration requiring stitches.

While assisting with the clean up around the shop, the employee began to sweat profusely and started feeling sick to his stomach. He went inside to cool off and didn't feel any better after drinking water and Gatorade. Employee was taken to the doctor where he received an IV and was released back to full duties with no follow up needed.

While conducting a pre-shift inspection, the employee climbed off of a front end loader and turned to go check the next piece of equipment and rolled his left ankle causing him to fall. The employee just received first aid treatment. On 9/30/14 his ankle was still bothering him so he was taken to the doctor where he was prescribed a steroid med and released back to full duties.

While ripping with Plant #87 bulldozer over rough terrain the employee noticed his shoulder was sore. (Employee said incident happened on 4/11/14 and he was taken to the doctor on 4/15/14).

Employee was operating a scraper when he ran over a large piece material that jerked the steering wheel pulling on his left shoulder.

Employee was stepping onto a trailer for servicing equipment when he heard his knee "pop".

Employee was removing the wheel hub on the #29 service truck. He gripped the front and rear of the hub. As the front hub cleared the axle housing in which it was supported, the weight suddenly shifted and the rear hub pinched his right middle finger between the hub and the studs.

The employee reached for a pry bar to help align the cylinder with the flange hole for placement of a pin. He took his eyes off his left hand which was on the side of the cylinder. His hand moved so his little finger was now resting on top of the cylinder. It is believed that the cylinder was bound up and it dislodged moving up approx. 1/4" pinching the employee's finger.

Employee was working from the top step of a 3 foot ladder. When he attempted to descend, he stepped off back side of ladder instead of the front. This caused him to fall, striking his right elbow against a metal table, causing a one inch laceration as well as swelling and bruising.

An employee was using a hammer to free a loader bucket tooth that was stuck. Pieces of metal broke off the tooth when struck by the hammer and struck nearby employees. The employee using the hammer had a 3/16 inch metal shard lodge in his upper left arm, and a nearby observing employee had a 3/8 inch metal shard lodge in his right hand.

An employee was using a hammer to free a loader bucket tooth that was stuck. Pieces of metal broke off the tooth when struck by the hammer and struck nearby employees. The employee using the hammer had a 3/16 inch metal shard lodge in his upper left arm, and a nearby observing employee had a 3/8 inch metal shard lodge in his right hand.

Employee was diagnosed with tendonitis in the right wrist that developed over time.

Grass fire started off of mine property near the 51-A highway and blew onto mine property. Extreme dry weather and windy conditions contributed to the fire. Canton and Fairview fire departments responded to our calls and extinguished the blaze. There was no damage. Employee called MSHA hotline for immediately reportable incidents at 3:15 pm on 2/16/09.

Employee sprained right knee upon stepping into fines pile while cleaning under apron feeder.

Mechanic placed his hand on a hot power washer muffler, resulting in 2nd degree burns to his right palm.

Employee was diagnosed with a mild case of carpal tunnel syndrome in left hand.

EE was using a ratchet with a 12" extension to remove a bolt from a converter on a loader. He exerted force to try and break the bolt loose. When it came free, the EE's finger struck the end of a screw clamp on a hose.

OPERATING SCRAPER OVER ROUGH TERAIN AND SPRAINED LOWER BACK.

EE WAS LEANING OVER A SCRAPER TIRE TO REMOVE STUDS FROM A BRAKE DRUM. AFTER SPENDING APPROX 4 HRS STANDING AND LEANING OVER WITH KNEE COMING IN CONTACT WITH THE TIRE, HE STARTED FEELING DISCOMFORT IN KNEE CAP

Employee was using screw driver to check for bearing noise. Screw driver contacted rotating shaft and handle was thrust in left ear.

Some metal cuttings got between the chuck and the drive on the lathe. Location was too tight to remove with pliers. EE tried using fingers and cutting sliced through his leather glove causing injury. Injury was laceration to the left index finger.

EMPLOYEE SAW A DOCTOR ON 1/28/04 STATING PAIN IN RIGHT ELBOW. DOCTOR DIAGNOSED DISCOMFORT AS TENDONITIS. EMPLOYEE STATES THAT NO INCIDENT OCCURRED. HE BELIEVES TENDONITIS IS DUE TO LIFTING 50 LB. BAGS ON 10/15/03. HE CLAIMS THAT THE PAIN BEGAN GRADUALLY OVER A PERIOD OF TIME AND NOT CAUSED BY A SPECIFIC EVENT. AFTER CONSULTING A PHYSICIAN, HE CLAIMS THE ILLNESS TO BE WORK-RELATED.

EE WAS ATTEMPTING TO REMOVE THE LID FROM A PLASTIC FIVE GALLON BUCKET SO IT COULD BE USED FOR TRANSPORTING TOOLS. HE CUT INTERLOCKING TABS WITH UTILITY KNIFE. AS HE APPLIED DOWNWARD PRESSURE, THRYING TO CUT THROUGH ONE OF THE TABS, THE BUCKET SHIFTED. HE REACHED TO GRAB IT AND INADVERTANTLY STRUCK HIS LEFT WRIST WITH UTLITY KNIFE.

THE INJURED EMPLOYEE AND CO-WORKER WEREIN THE PROCESS OR REPAIRING A D11 DOZER THAT HAD BROKEN DOWN IN #2 PIT. A FINAL DRIVE WAS BROUGHT IN BY FLATBED TRAILER. USING A BOOM TRUCK, THE FINAL DR IVE WAS REMOVED, POSITIONED AT THE END OF THE TRAILER, AND BLOCKED TO PREVENT SIDE-TO-SIDE MOTION. AS THE CO-WORKER STEPPED OFF, HE FELL TO THE GROUND, LANDING IN UNEVEN AREA.

EMPLOYEE WAS OPERATING 707 SCRAPER UP A RAMP AFTER RETURNING FROM THE SPOIL PILE. HE WAS TRAVELING IN 6TH GEAR WITH THE EASY RIDE DISENGAGED. AT THE EDGE OF THE RAMP THE SCRAPER ENCOUNTERED SO ME ROUGH TERAIN, JARRING THE EMPLOYEE'S BACK.

EE WAS IN THE PROCESS OF REMOVING PIN WHICH ATTACHES RIPPER FRAME TO THE #88 D11 DOZER. A HYDRAULIC WEDGE WAS BROUGHT IN TO ASSIST IN THE PIN REMOVAL. AS THE EE BEGAN APPLYING PRESSURE TO THE WEDGE, IT DISLODGED STIKING THE EE IN THE MOUTH.

EE WAS CHIPPING A WELD WITH AN AIR CHIPPER AND HAND CHIPPER WHEN A PIECE OF SLAG CIRCUMVENTED HIS SAFETY GLASSES.

EE WAS IN THE PROCESS OF REMOVING THE FRONT BELLY PAN ON 739 DOZER TO REPAIR A COOLANT LEAK. THE BELLY PAN JACK WAS HANGING UP AND NOT COMING DOWN. EE TRIED USING A PRYBAR TO ASSIST. THE BAR S LIPPED & HIS FORWARD MOMENTUM CAUSED HIM TO STRIKE HIS MOUTH ON THE BLADE STABILIZER BAR.

EE AGGRAVATED A PRE-EXISTING CONDITION WHILE HE WAS STANDING ON A WEAR PLATE THAT PROTRUDED FORM THE HOPPER WALL USING A TORCH TO CUT OUT SOME CONNECTION BOLTS ON THE EAST GATE CRUSHER HOPPER. HE WAS STANDING ON A WEAR PLATE THAT PROTRUDED FROM THE HOPPER WALL AND WAS STEADYING HIMSELF BY HOLDING ONTO VERTICAL BEAM WITH HIS LEFT HAND, ARM EXTNEDED AT SHOULDER LEVER. WHILE CUTTING A

ON 5/25/00, EMPLOYEE REPORTS HE BUMPED HIS ELBOW ON A SCRAPER SEAT. NO TREATMENT WAS RENDERED. THEN ON 7/26/00, HE REPORTS AGGRAVATING SAME ELBOW INJURY WHEN USING A SLEDGEHAMMER & ASKED TO SEE A DOCTOR. HE WAS TAKEN TO CLINIC ON 7/28; X-RAYS TAKEN WERE NEGATIVE. ANTI-INFLAMMATORY WAS PRESCRIBED AND THE EMPLOYEE WAS RELEASED TO RETURN TO WORK.

EMPLOYEE WAS POSITIONED UNDER THE JAW CRUSHER SHIM GUARD TO CATCH BOLTS THAT WOULD FALL ONCE PLATE WAS RAISED OUT; HE HAD HIS HAND ON THE WELDED NUT OF THE BOLT WHEN THE BOLT MOVED UP, PINCHIN G HIS FINGER BETWEEN THE BOLT HEAD & THE BOTTOM OF THE JAW CRUSHER.

WHILE CUTTING THE CHAIN ON THE MAIN CRUSHER APRON FEEDER, EE WAS STANDING ON I BEAM (APPRX 18" HIGH) WITH LEFT FOOT ON BEAM AND OTHER ON THE WEB. HIS RIGHT FOOT SLIPPED AND HIS RIGHT SHIN HIT THE TOP OF THE BEAM JUST BELOW THE KNEE.

EMPLOYEE WAS WORKING UNDERNEATH #713 GRADALL WITH VERY LITTLE HEAD ROOM. HE WAS ATTEMPTING TO LOSOEN BOLT WHE HIS RATCHET SLIPPED. HE COULD NOT MOVE IN ANY DIRECTION TO AVOID WORKING IN FRON T OF FACE. THE RATCHET STRUCK HIM IN THE MOUTH INFLICTING THE INJURY.

EE WAS HELPING PREPARE D10 DOZER TRACKS FOR LOADING AND SHIPMENT ON CONTRACTOR'S TRUCK. THE TRACKS HAD BECOME MISALIGNED AND WOULDN'T FOLD PROPERLY. THE INJURED AND ANOTHER CO-WORKER LIFTED UP ON EACH SIDE TO READJUST. THE INJURED'S HAND WAS PLACED BY A GAP BETWEEN THE PADS OF THE TRACKS. WHEN THEY LIFTED, THE BINDED TRACKS SLAMMED DOWN. CLOSING THE GAP AND PINCHING HIS HAND.

EE HAS DEVELOPED CARPAL TUNNEL SYNDROME ATTRIBUTED TO OPERATION OF HEAVY EQUIPMENT OVER TIME. THE LEFT WRIST WAS SURGICALLY REPAIRED ON 9/7/99 AND THE RIGHT ARM ON 11/11/99.

EE WAS ATTEMPTING TO DISLODGE THREE ROCKS THAT WERE WEDGED IN THE CHUTE LEADING FROM THE MAIN HOPPER OF THE ROAD ROCK CRUSHER. HE LOCKED OUT THE EQUIPMENT AND INITIALLY TRIED TO CLEAR THE JAM WITH A PRY BAR. WHEN THIS DID NOT WORK, HE PROCEDED TO MANUALLY TRY TO MOVE THE ROCKS WITH HIS HANDS. HE INCURRED THE INJURY AS HE TWISTED HIS BODY AND ATTEMPTED TO LIFT.

THE EE WAS OPERATING A CAT 657 SCRAPER. WHILE MAKING A RETURN PASS TO THE CUT, THE SCRAPER BEGAN BOUNCING SEVERELY ON THE TERRAIN. THE SCRAPER TRAVELLED DOWN THE MAIN STRIP ROAD ABOUT 185' FRO M WHERE THE OPERATOR HAD RAISED THE BOWL. IT THEN PROCEEDED ANOTHER 65' BEFORE COMING TO A STOP. DURING THIS DISTANCE, THE BOUNCING ACTION OF THE MACHINE SHOOK THE EE INFLICTING THE INJURY.

EMPLOYEE WAS ATTEMPTING TO OPEN THE RIGHT HAND WINDOW IN THE FRONT CAB OF 718 GRADALL. HE ENCOUNTERED SOME RESSISTENCE DUE TO A BUILD UP OF DIRTIN THE WINDOW TRACK. HE APPLIED ADDITIONAL FORCE & THE WINDOW OPENED ABRUPTLY SMASHING HIS THUMBBETWEEN THE WINDOW LATCH AND THE INNER WINDOW FRAME.

EMP WAS IN PROCESS OF TRYING TO PICK UP 1'X 2'X 3/4" THICK SCRAP METAL PLATE FROM SHOP FLOOR. SINCE PLATE WAS LYING IN POOL OF OIL, EMP ELECTED TO USE A GASKET REMOVER (FLAT BLADE SCREWDRIVER) IN RT HAND TO PRY IT LOOSE. WHILE HOLDING TOOL IN RT HAND AND APPLYING OPPOSITE PRESSURE WITH LEFT HAND,SCRAPER SLIPPED, COMING ACROSS THE PLATEWHERE IT STRUCK EMPS LEFT WRIST.

WHILE WALKING ALONG THE DOZED PATH BETWEEN THE SOUTH NIPPERT AND CHARLIE PONDS, AN UNKNOWN INSECT BIT ARM.

WHILE RETURNING A PIECE OF PIPE TOA PIPE STAND, PORTABLE PLATFORM LADDER EMPOYEE WAS STANDING ON TIPPED OVER. HIS LEG WAS SCRAPED BY A PIECE OF STEP GRATING ON THE PLATFORM.

EE WAS ASSIGNED TO HELP THE DRILLER ON THE CORE RIG. HE WAS IN THE PROCESS OF ADDING A LENGTH OF PIPE. UPON LOOSENING A CONNECTION WITH A PIPE WRENCH, THE PIECE OF PIPE BROKE FREE AND POPPED U P DUE TO TENSION ON THE LIFT CABLE. HIS THUMB BECAME PINCHED BETWEEN THE JOINT BOLT ON THE PIPE TOOL AND THE BOTTOM OF THE KELLY DRIVE INFLICTING THE INJURY.

EE HAD JUST FINISHED UNHOOKING A CRAN CABLE FROM A CONVEYOR BELTLINE. AS HE STARTED TO WALK BACK DOWN THE CATWALK, A CABLE GUIDE (BENT STEEL BAR) ON TOP OF THE CRANE BOOM BROKE LOOSE AND FELL. IT INITIALLY STRUCK EE'S HARD HAT AND THEN GLANCED OFF HIS SHOULDER BEFORE FALLING TO THE GROUND.

THE EE WAS IN THE PROCESS OF DUMPING HIS SCRAPERWHEN IT BEGAN TO BOUNCE OVER THE TERRAIN. HE WASTHRUSTED INTO THE DOOR FRAME OR ROLLOVER FRAME STRIKING THE BACK OF HIS HEAD & THEN PROCEEDED TO CAROM FORWARD HITTING THE TOP OF HIS HEAD ON THEINSTRUMENT PANEL. [NOTE: EE WAS WEARING A SEAT [LAP] BELT.]

EMPLOYEE WAS IN THE PROCESS OF EMPTYING A BAG OFANFO INTO A WHEELBARROW. THIS IS DONE BY STRIKING THE BAG AGAINST THE WHEELBARROW BOWL TO BREAK IT OPEN. AS HE LOWERED THE BAG TO DO SO , HIS HAND STRUCK A SHARPENED DYNAMITE PUNCH WHICH IS AFFIXED TO THE WHEELBARROW HANDLE.

EMPLOYEE HAS COMPLAINED OF GRADUALLY DEVELOPING SHOULDER PAIN OVER THE LAST FEW MONTHS WHENHE IS ASSIGNED TO OPERATE A LOADER (IE. STEERING CONTROLS VIBRATION SOUGHT MEDICAL TREATMENT AS SYMPT OMS PERSISTED

WHILE WELDING ON 702 SCRAPER BOWL, EMPLOYEE WIPED FACE AND GOT A PIECE OF METAL IN HIS RIGHT EYE.

EMPLOYEE WAS WALKING OUT TO A HAULAGE TRUCK WHEN HE SLIPPED ON SNOW AND ICE, FALLING TO THE GROUND. INITIAL DR. APPOINTMENT ON 1/3/95, NO TREATMENT RENDERED. EMPLOYEE STILL EXPERIENCED DISCOMF ORT AND WAS RE-EXAMINED BY THE DR. ON 1-13-95 AT WHICH TIME A PRESCRIPTION FOR PAIN WAS DISPENSED AND THE INCIDENT BECAME REPORTABLE.

DURING DOWNTIME IN THE CRUSHER, WHILE STANDING ON THE JAW LEVEL FLOOR, AMETAL PLATE WHICH WAS LEANING AGAINST A SAFETY RAIL, FELL FORWARD STRIKING THE BACK OF HIS LEG.

EMPLOYEE WAS STEPPING OFF THE LADDER OF 706 TRUCK & PLACED HIS LEFT FOOT ONTO A RIDGE OF CRUSHED GYPSUM FINES ANKLE ROLLED TO THE RT CAUSING THE SPRAIN MEDICAL TREATMENT SOUGHT O N0 6-23-94 A S A RESULT OF PERSISTENT SWELLING

THE EMPLOYEE WAS INT HE PROCESS OF LOADING A SHOT. HE WAS USING A WHEELBARROW TO TRANSPORT ANFO. A GUST OF WIND TIPPED THE WHEELBARROW. THE EMPLOYEE REACHED OUT QUICKLY TO TRY AND STEADY IT . IN DOING SO, HIS FINGER CAME IN CONTACT WITH A KNIFE SETTING IN A HOLDER ON TOP OF THE WHEELBARROW BUCKET. THE KNIFE IS USED TO CUT DETONATION CORD.

FOOT SLIPPED WHILE WALKING AND HE FELL AGAINST HAND RAIL AT TAIL SHAFT AREA OF THE BELT CRUSHER

THE EMPLOYEE WAS ATTEMPTING TO LOWER THE LEFT WINDOW ON A HAUL TRUCK. T HE REGULATOR FAILED CAUSING THE WINDOW TODROP SUDDENLY AND SMASH HIS THUMB BETWEEN THE WINDOW LATCH AND FRAME.

EMPLOYEE REPORTS THAT WHILE OPERATING THE CAT 367E SCRAPER HIS LEFT KNEE WOULD SOME TIMES STRIKE THE STEERING COLUM WHICH HAD 2 SCREWS PROTRUDING FROM IT. OVER A PERIOD OF TIME, THIS ACTION CA USED A BUILDUP OF FLUID IN THE OPERATOR'S LEFT KNEE CAP.

EMPL TRYING TO FIGURE WHY #1 WHITE ELEVATOR FEED BELT WAS CYCLING ON AND OFF...EMPL PROCEEDED TO CHECK BELT TENSION WITH HAND,EQUIP RESTARTED DRAWING FINGER INTO NIP POINT CREATED BY BELT AND PULLEY.

AIR SYSTEM TO PUMP DRIVE ACTIVATED, NOZZLE VALVE STUCK OPEN WHEN PLACED IN CARRIER DIESEL FUEL BURPED IN SPLASHED IN EMP EYE

EMPLOYEE WAS OPERATING A DROPBALL CRANE. WHILE IN THEPROCESS OF PICKING UP AND BRINGING THE BALL INTO DROP IT AGAIN, THE HOIST CABLE BROKE. THE BALL FELL TOTHE GROUND, SHATTERING THE FLOOR ROCK. A SMALL PARTICLE OF THE SHATTERED ROCK PENETRATED THE EMPLOYEES EYE FROM THE SIDE.

EMPLOYEE WAS OPERATING #716 SCRAPER. HE REPORTS THAT WHILE LOOKING BACK OVER HIS SHOULDER AT THE LOADING PROCESS, HE STRUCK SOME ROUGH TERRAIN WHICH RESULTED IN HIM TWISTING HIS NECK.

THE EMPLOYEE HAD JUST DESCENDED A HILL ON A LOADED SCRAPER THE PIT FLOOR AT THE BASE OF THE HILL HAD SOME CREVICES WHILE DRIVING ACROSS THIS AREA THE EMPLOYEE REPORTS THAT HE WAS JERKED ABOUT IN THE CAB AND SUDDENLY FELT A SHARP PAIN IN HIS LOWER BACK.
