The employee was lowering a guard in to position and the guard got hung up while it was being lowered. The employee had hand positioned on the bottom section of the guard and when the guard freed up, it pinched the employee's right index finger, causing a hairline fracture to the finger.

A wildfire that started near Hitchcock (Blaine County 720 Fire) burned onto USG property. When it reached the property, we notified MSHA. It burned several acres on the mine site. No personnel were injured and no equipment was damaged.

Employee was repositioning a jack-stand, and while maneuvering the stand, it shifted and the employee placed EE's right hand on the top of the stand to prevent it from tilting forward, resulting in pinching EE's right thumb in between the stand and the frame of mobile equipment EE was working on. The tip of the right thumb was fractured.

Employee hit ruts, causing employee to bounce and seat suspension bottomed out, jarring employee's back. Employee was taken to clinic and released.

Operator just dumped a load in the dump area from the CAT 657G scraper. As they were leaving the dump area, they drove through an area of roughness as they were lowering the apron. The rough ground jarred the scraper, causing the operator's left shoulder to strike the safety bar on the left side of the operator's cab causing a fracture to left clavicle.

Employee was removing an elevator chain pin that was wedged between the idler and elevator casing. While pulling the pin, the employee placed right hand over the end of the pin. Another mechanic who was assisting in this job was jogging the elevator. The elevator rolled back wedging the pin against the floor and the employee's right hand.

Employee was disconnecting and rolling up 4" flat discharge hose from a trash pump. As employee attempted to unlatch the QD fitting, the right hand was cut by the end of the band-it clamp tail causing a laceration on base of right thumb. Employee saw doctor and received 2 stitches.

EE was operating the loader, the air conditioner did not work so EE opened up the windows for air. At 2:00 pm left eye was irritated. In the morning when EE woke up the eye was almost swollen shut. EE was taken to doctor (5/27/16) where the doctor did not find anything in the eye. The doctor diagnosed as allergy, gave employee steroid eye drops and released employee to work.

Two employees were removing a drill steel from a storage rack. As they pulled the drill steel towards themselves, the other end came off the racking storage arm. This caused the up swinging end to pinch the injured employee's hand against the underside of the next level racking arm, causing a laceration/avulsion to left hand ring finger base knuckle.

Quarry mechanic was backing a portable 4" pump into the Quarry Crusher Maintenance shop. When the mechanic released the pump trailer hitch from the pickup bumper ball, the pump trailer moved forward, pinching his right index finger between the trailer hitch and rear bumper. Employee had a laceration requiring stitches.

While assisting with the clean up around the shop, the employee began to sweat profusely and started feeling sick to his stomach. He went inside to cool off and didn't feel any better after drinking water and Gatorade. Employee was taken to the doctor where he received an IV and was released back to full duties with no follow up needed.

While conducting a pre-shift inspection, the employee climbed off of a front end loader and turned to go check the next piece of equipment and rolled his left ankle causing him to fall. The employee just received first aid treatment. On 9/30/14 his ankle was still bothering him so he was taken to the doctor where he was prescribed a steroid med and released back to full duties.

Employee was removing the wheel hub on the #29 service truck. He gripped the front and rear of the hub. As the front hub cleared the axle housing in which it was supported, the weight suddenly shifted and the rear hub pinched his right middle finger between the hub and the studs.

The employee reached for a pry bar to help align the cylinder with the flange hole for placement of a pin. He took his eyes off his left hand which was on the side of the cylinder. His hand moved so his little finger was now resting on top of the cylinder. It is believed that the cylinder was bound up and it dislodged moving up approx. 1/4" pinching the employee's finger.

Employee was working from the top step of a 3 foot ladder. When he attempted to descend, he stepped off back side of ladder instead of the front. This caused him to fall, striking his right elbow against a metal table, causing a one inch laceration as well as swelling and bruising.

An employee was using a hammer to free a loader bucket tooth that was stuck. Pieces of metal broke off the tooth when struck by the hammer and struck nearby employees. The employee using the hammer had a 3/16 inch metal shard lodge in his upper left arm, and a nearby observing employee had a 3/8 inch metal shard lodge in his right hand.

An employee was using a hammer to free a loader bucket tooth that was stuck. Pieces of metal broke off the tooth when struck by the hammer and struck nearby employees. The employee using the hammer had a 3/16 inch metal shard lodge in his upper left arm, and a nearby observing employee had a 3/8 inch metal shard lodge in his right hand.

Grass fire started off of mine property near the 51-A highway and blew onto mine property. Extreme dry weather and windy conditions contributed to the fire. Canton and Fairview fire departments responded to our calls and extinguished the blaze. There was no damage. Employee called MSHA hotline for immediately reportable incidents at 3:15 pm on 2/16/09.
