WHILE LOWERING THE SCREENER WITH A LOADER THE INJURED EE WENT TO UNHOOK THE CHAINS ON SCREENER, THOUGHT THE SCREENER WAS ON THE GROUND, SO TOLD LOADER OPERATOR TO LET SLACK OUT OF CHAINS, RESU LTING IN SCREENER COMING, RESTING ON HIS RIGHT FOOT.

CHANGIN A BEARING ON THE TAIL PULLEY BREAKING LOOSE THE BOLTS WITH A BREAKER BAR. WHEN THE BOLT CAME LOOSE HE BUMPED HIS ELBOW ON THE TAIL PULLEY COVER DOOR.

REPLACING SAND SCREEN ON SCREENER, SLIPPED AND FELL AGAINST THE STEEL FLASHING
HAD JUST REPLACED THE STARTED ON THE SCRAPER AND WAS REPLACING THE COVER WHEN IT SLIPPED ADN THE EDGE OF THE COVER CAUSED A LACERATION BETWEEN THE THUMB AND INDEX FINGER OF THE LEFT HAND.
EE WAS GETTING DOWN AFTER WELDING A BEAM. WHILE DOING SO HIS WELDING STICK CAME INTO CONTACT WITH THE BEAM AGAIN CAUSING A FLASH.
