EMPLOYEE WAS HOOKING UP SEMI-TRACTOR TO A GOOSENECT TRAILER. THERE WAS A CHAIN HOOKED THRU A D-RING ON THE SIDE OF THE TRAILER. THE WELD ON TEH D-RING BROKE AND THE CHAIN WHIPPED AROUND AND STRUCK EMPLOYEE ON RIGHT SIDE OF HEAD ABOVE AND BEHIND RIGHT EAR LACERATIONS AND CONTUSIONS TO SCALP.
