While helping to lift a wooden crate, a board came loose. He lost his balance and fell to the ground on left wrist.

EE was removing a jammed piece of concrete from the collector belt of the gravel in-feed bin. The concrete fell and crushed his left index finger against the angle iron frame of the bin causing a laceration.
He had attached a hook and rope to the bottom screen in order to pull it out through the top. The hook slipped and his right elbow struck against the frame of the screen deck. No expected impairment.

Breaking up a large piece of concrete with a jack-hammer. The hammer broke through and a chunk of concrete caught the small finger of his left hand.
EE noticed that the shackle connecting the cable to the cutter head was not properly installed and proceeded to re-install the shackle. He was standing on the ground in front of the cutter head assembly, when the assembly dropped approximately four inches. One of the tines of the cutter head struck the front of his left leg, causing an abrasion and contusions.

EMPLOYEE GOT SOMETHING IN HIS EYE THAT SCRATCHEDTHE SURFACE OF HIS EYEBALL. WHEN QUESTIONED HE STATED THAT HE HAD BEEN CUTTING METAL, BUT HAD HIS SAFETY GLASSES ON. HE STATED HE MAY HAVE GOTTEN IT FROM BLOWING DEBRI.
EMPLOYEE WAS CUTTING A PIECE OF OLD CONVEYOR BELT WITH A UTILITY KNIFE. HE LET THE KNIFE SLIP AND CUT HIMSELF ON THE LEFT HAND BETWEEN HIS THUMBAND FIRST FINGER. INJURY REQUIRED TWO STITCHES.
EE WAS CLEANING THE WASHER (GRAVEL) AND SLIPPED ON A WET SPOT FROM HIS USE OF A WATER HOSE.
