Individual injured while helping change out a bucket elevator belt. As the old and new elevators were being replaced, they got stuck together, causing them to bind. The tension caused them to abruptly come apart, striking individual on right hand thumb area causing a fracture to the thumb. Injured party was transported to get evaluated, x-rays taken confirming fracture to thumb.
While placing a diester insert and attempting to maneuver in place, IP went to grab a tool and as placed right hand along the top railing area to maintain three points of contact, the suspended load shifted as it was shimmied in place, causing load to come down approx. 3-6" on hand, causing lacerations to top of fingers, and comminuted fractures to ring and pinky fingers.

A dry plant employee had gone to the head box of bucket elevator 175A to clear an obstruction. When completed the employee had leaned the shovel against the hand rail and started to descend down. Upon starting the bucket elevator the shovel had fallen and struck another dry plant employee on the head. This resulted in a light concussion. Dr. had ordered one day off for recovery.

Employee was using EE's personal knife to cut a zip tie and cut EE's left thumb. EE was not wearing gloves, and was pulling the knife towards EE's body. When EE cut through the zip tie the knife struck EE's left thumb. This resulted in an approximately 1 inch laceration which took 6 stitches to close laceration.
Employee slipped while climbing down 10' A frame ladder, EE's left foot slipped off rung resulting in a laceration to EE's back left thigh from a piece of angle iron that was behind EE. Laceration was closed with 21 stitches.
