WHILE MOVING A FEEDER GATE LITTLE FINGER ON LEFT HAND WAS CAUGHT BETWEEN THE TUNNEL WALL AND FEEDER 4 MEN WERE HANDLING THE FEEDER TO TURN IT AROUND WHEN IT CAUGHT ON THE CONVEYOR AND PIVOTED CATCHING HIS FINGER AND CAUSING A LACERATION

EMPLOYEE WAS DRIVING A SERVICE TRUCK AND ROAD CONDITIONS WERE HAZARDOUS AND HE WAS DRIVING TOO FAST FOR CONDITIONS HE RAN A STOP SIGN AND HIT AN EMBANKMENT. HE COMPLAINED OF SORENESS IN THE NE CK ON THE NEXT DAY 10-29-88 AND WENT TO DR FOR TREATMENT.

THE INJURED AND ANOTHER EMPLOYEE MOVING A 380# PIECE OF IRREGULAR SHAPED PLATE STEEL FROM A FAB TABLE AND SETTING IT DOWN ON THE FLOOR.OTHER EMPLOYEE SET HIS END DOWN AND WENT TO HIS SIDE TO S TAND PLATE UP ON ITS END AND THE INJURED LOST HIS GRIP AND DROPPED THE PLATE APPROXIMATELY 18" ONTO THE BRIDGE OF HIS LEFT FOOT.
