VICTIM STEPPED ACROSS CONVEYOR BELT TO SHUT OFF PUMP WHEN BELT WAS STOPPED FROM INSIDE HIS FOOT BECAME ENTANGLE IN BELT AND BELT WAS ACTIVATED INSIDE MINE, BEFORE HE COULD FREE HIMSELF, RESULT IN HIM BEING PULLED INTO DRIVE UNIT WHERE UPON HE LOST HIS RIGHT FOOT.
