Employee was climbing up onto the back of utility truck. Said employee lifted their left leg up onto bed of the truck then used both arms pull the rest way up. Employee said they felt pain in left shoulder.

Hydraulic hose broke in engine compartment of Ed's Drilling & Blasting rock drill. The resulting fire destroyed the engine compartment and operator's cab. The operator off the drill used fire ext. and then evacuated the area. No injuries occurred during this event. No quarry employees were involved.

A rock was stuck in the jaw crusher. EE entered the crusher to break the rock free. The area couldn't be reached with the breaker arm. As the rock broke free it rolled on another rock & struck his knee. He was able to get out & just complained of a bruised knee. After tool clean up the supervisor asked to see the knee & recommended he go to the hospital to check it out.
