employee was performing a prework inspection of a rotary crusher. while rotating the drum to each hammer EE's left hand became caught between the wall and hammer. resulting in fracture of EE's left pinky finger. referred to an orthopedic surgeon.

Electrician was checking voltage on new circuit breaker when it started making a noise and the started arching. When arching occurred there was a large flash causing 2nd degree burns to the electricians right thumb and forefinger. Electrician was treated and released back to work the same day.

The men were hoisting a 50hp electrical motor up to the 2nd level platform with a boom truck when the chains came loose from the motor causing the motor to come loose falling approx 4' smashing EE left foot

Operator was driving a haul truck when he ran over a bump in the road causing him to bite the inside of his lip
The employee was attempting to free up a curtain on a secondary crusher that had gotten hung up in the open position. The employee freed the curtain causing it to free fall smashing his left hand fifth finger
The employee was removing steel anvils from a crusher ring using a #10 sledge hammer when the anvil shattered and a piece of it struck him in the forehead about an 1" above the left eye. The employee nor the foreman working beside him were wearing their PPE.

The operator backed into a concrete wing wall at the railcar loading ramp causing a puncture in the sidewall which resulted in the tire blowing out. The operator recieved a strain to her left thumb and wrist while firmly holding on to the steering wheel of her haul truck.

Fracture and laceration to the distal tip of the right 5th digit from being caught between an electrical transformer and bed of truck.
Employee was changing the oil on a rock truck and started to feel dizzy. Climbed into cab of oil truck to cool down and passed out. Employee was found unconscious and was taken to hospital. He was admitted overnight for observation.

Foley Caterpillar mechanic while working on the exhaust system of a rock truck was observed by quarry personnel to take a break for shade and water. When next observed, the mechanic was lying down. The quarry personnel stopped to check mechanic; 911 was called. Health conditions determined heart failure.
