While bagging 40# bags of coal, employees hand slipped and struck metal clamp. Employee was taken to local Urgent Care for observation, received four stitches to LEFT thumb and diagnosed with laceration without foreign body.
Employee while walking toward portable restroom (Job Johnny) located at Slate Hopper area, slipped losing footing causing fall onto RIGHT side and felt pain/soreness to RIGHT shoulder area. Employee was taken to local Urgent Care and diagnosed with sprain to RIGHT shoulder.
While walking to load next retail truck in loadout section of #14 Prep Plant, employee's wet boot slid on floor causing RIGHT ankle to "twist" while attempting to change direction. Employee was taken to local Urgent Care for observation and diagnosed with sprain to RIGHT ankle.

Employee stated on 3/07/24 while bagging coal and working on restacking pallets of damaged 40# bags of coal when employee felt soreness to RIGHT shoulder area. The next morning on 3/08/24 employee reported incident, was taken to local Urgent Care for observation and diagnosed with strain of muscle and upper arm of RIGHT shoulder area and given restrictions.
On 1/25/24 employee descended wheel loader #LO041, planted feet on ground and slipped. Employee then grabbed ladder w/ RIGHT hand to prevent slip/fall causing soreness to RIGHT arm area. Later on 1/29/24 employee went to local Urgent Care for observation and diagnosed with sprain to RIGHT acromioclavicular joint and placed on light duty.

Employee was moving material while operating wheel loader, drove over a pot hole, immediately applied foot brake causing loader to come to sudden stop. Employee felt a strain in lower back area and was taken to local Urgent Care for observation and diagnosed with strain of muscle to lower back and placed on light duty.
While attempting to spread apart and adjust forks on Cat TL943 Telehandler (Asset #FL002), employee felt pain in RIGHT shoulder and neck area. Employee was taken to local hospital for observation and diagnosed with strain of muscle.

Making repairs to discharge lip on Unit #708 vibrator, employee stepped over center divider and felt cramping/pain in R. thigh area. Employee reported incident on Monday 10/31/22 and went for observation and was treated for back pain with radiculopathy.
On 6/29/2022 employee was replacing vibrating unit. While grinding old weld employee reported EE felt something fall in EE's right eye, refused medical observation. Then on 7/05/22 employee complained of irritated eye. Went to local Urgent Care for observation and diagnosed w/ foreign body in right eye. Sent to eye doctor and had sliver removed.
A full 40# bag of coal slipped out of hands, employee tried to lift upwards to prevent bag from falling to floor, hands slipped on wet bag and left hand impacted bottom of metal chute causing laceration to left thumb. Employee was taken to local Urgent Care and diagnosed with laceration of left thumb.
Employee incorrectly placed an empty bag on bagging machine and reached to attempt to straighten bag when their left thumb impacted metal clamp causing laceration to left thumb. Employee was taken to local Urgent Care for observation and diagnosed with a laceration without foreign body of left hand.

Miner was loading packaged blasting agent and shoveling (stemming) bore holes and noticed EE's shoulder getting sore. Applied ice for several days with no relief. Reported the injury on 08/05/2021. Sought medical care on 08/09/2021.
While accessing repairs needed in Buckwheat pocket employee slipped, reached with LEFT hand to grab onto something preventing fall and cut LEFT middle and ring fingers on piece of metal. Employee was taken to local hospital for observation and diagnosed with LEFT middle and ring finger laceration.
While shoveling, prepping and cleaning out the inside of a railcar employee slipped and fell. EE reached out with LEFT arm to catch self, arm twisted and they fell on left side causing pain and soreness to LEFT shoulder area. Employee was taken to local Urgent Care for observation and diagnosed with sprain to LEFT shoulder area and placed on restrictive duty.
While cleaning up coal bins at bagging department employee was shoveling coal and felt soreness to lower back area. EE was taken to local Urgent Care for observation and diagnosed with muscle strain to lower back and placed on restricted duty.

While loading trailer on 11/18/2020 with 2,000lb. skids of bagged coal, employee attempted to pull pallet with pallet jack which did not move and felt a pull and pain to left elbow area. Initially employee refused medical treatment. On 11/20/2020 employee was taken to local Urgent Care for observation and diagnosed with strain of left elbow and placed on restricted duty.
On 10/07/2020 employee reported while setting a 12" 3'L x 4'L 90 degree steel pipe elbow employee turned left to re-position self and felt a pain to lower right back area and initially refused medical treatment. On Monday 10/12/2020 employee was still sore and went to local Urgent Care and diagnosed with a muscle strain and placed on restricted duty.
On 7/22/2020 while turning a 12" x 21'L piece of steel pipe with beam turner employee reported EE slipped on wet floor and felt strain in lower back area but refused any medical treatment. On 10/16/2020 employee was taken to local Urgent Care for observation after complaining area was still sore and diagnosed with muscle strain and given restricted duty.
While assisting packaging coal, employee was placing 40# bags of coal into sealer and felt pain and soreness in left arm/shoulder area. On July 7, 2020 employee's arm was still sore and employee was taken to local Urgent Care and diagnosed with pain in left shoulder. Employee was given lifting restrictions and currently working on restricted duty.
While descending steps and carrying ladder in plant, employees right foot twisted causing foot to twist and become sore. Employee finished shift. The next day employees foot was still sore and was taken to local Urgent Care for observation and diagnosed with a sprained ligament.
Employee stated on Thursday 2/13/2020 morning they developed right shoulder and neck soreness about a week ago while bagging nut coal and now wants to report it. Employee was taken to local Urgent Care for observation and diagnosed with strain in right shoulder area and placed on restricted duty.
While placing empty bag onto bagging chute employee felt soreness in right shoulder. Employee was taken to local Urgent Care and diagnosed with right shoulder strain and placed on restricted duty.
Employee was removing 40# bags of coal off stacking conveyor and as EE pulled bag and attempted to place on pallet EE felt a pinch in EE's left shoulder area. EE was taken to local urgent care for observation and x-ray. Employee was diagnosed with left shoulder strain.
Employee went to grab a 40lb. bag of coal that fell off bagging chute, and felt pain in right shoulder area. Employee was taken to local Urgent Care and diagnosed with strain to right arm and placed on restricted duty.

On 12/14/19, employee was loading retail coal, pulled lever to close hopper bin, stood up from folding chair and felt pain in lower back area. Employee refused treatment. On 12/30/19, employee was still sore and taken to local Urgent Care for observation, and diagnosed with lower back strain.
While taking 40# bag of coal off stacking table, employee turned to right and attempted to place 40# bag of coal on pallet and felt pain in right shoulder area. Employee was taken to Urgent Care and diagnosed with strain in Right Shoulder.
While applying hand brake during positioning of railroad car, employee felt strain and soreness in lower back area. Employee was taken to local Urgent Care for observation and diagnosed with muscle strain.
On 11/01/19 employee was performing maintenance on Ripl-Flo vibrator, stepped on spring base pedestal, boot slipped and Right Knee impacted metal pedestal causing pain. Initially employee refused medical attention. Knee was still sore and employee went for MRI on 12/07/19 and diagnosed with possible hairline impact fracture.
On 7/01/19 employee was stacking 40lb. bags of coal onto pallet and felt pain in left knee area, informed lead man but refused to go for medical attention. Later on 7/08/19, employee stated the left knee was still sore and was taken to local Urgent Care for observation and diagnosed with pain in left knee and placed on restricted duty.
Employee was changing screens on vibrator and felt pain in the right elbow area. Initially they refused treatment. On 6/25/19 soreness and pain persisted and employee was taken to local Urgent Care for observation and diagnosed with tendinitis in right forearm and placed on restricted duty.
Employee was working re-positioning pallet of bagged coal in shipping trailer. When the employee attempted to pull skid using pallet jack, wheel of pallet jack mobility became hindered by dirt/debris on floor of trailer causing resistance. Employee felt soreness and pain in lower back area & was taken to local Urgent Care for observation & diagnosed with lower back pain.

Employee was lifting metal screens, while bending over to lift a second screen employee felt pain and soreness in lower back area. Employee was taken to local Urgent Care for observation and diagnosed with a muscle strain.
Employee reported Monday 3/05/18 that while performing maintenance on Thursday 3/01/18 EE slipped and grabbed a hold of railing with left arm to prevent falling to ground. On 3/05/18 left bicep was swollen and sore. Employee was taken to local Urgent Care for observation and diagnosed with bruised left bicep muscle.
Working packaging bagged coal, employee attempted to clean spilled coal off stacking table w/ right hand. Top of right hand brushed against automatic bag sealing machine discharge area causing abrasion to back of right hand. Employee was taken to local Urgent Care 2/09/18 for observation and x-ray, and diagnosed with abrasion to right hand.
While pulling in a downward motion attempting to open manual overhead garage door, employee felt soreness in back area. Employee was taken to local Urgent Care for observation and diagnosed with a strain of muscle and tendon of back wall of thorax.

While entering lab, employee stumbled on metal grating and fell forward using both hands to brace fall which caused cuts to both hands and finger. Employee was taken to local Urgent Care for observation and diagnosed with laceration without foreign body to both hands and finger and received a total of 13 stitches.
On 4/07/17 employee reported a strain/pain to right shoulder area while pulling lever down in load out area of #14 Plant and refused medical attention. Later on 06/16/17 same employee reported pain still present and taken to local Urgent Care for observation and diagnosed with strain of muscle to right shoulder.

While observing recently repaired conveyor, employee placed hand near scraper and bend pulley to look at belt tracking. A piece of ice on bend pulley came around and pinched right third finger between ice and scraper causing laceration to tip of finger.
A foreman loosened a nylon ratchet strap and the metal handle of ratchet strap struck EE's right hand causing a cut. Employee was taken to local Urgent Care for observation and diagnosed with a finger laceration to EE's right hand which required stitches.
While employee was lifting metal screen, they felt soreness and strain in their lower back section when they attempted to stand up. Employee was taken to local Urgent Care and diagnosed with a lumbar strain.

Employee was attempting to open/turn wheel of water valve with excess force which caused pain to left shoulder, wrist and arm area. Employee was taken to local hospital for observation and diagnosed with left shoulder strain.
A co-worker slipped and lost balance while performing maintenance on 12" x 10" pump and injured employee attempted to break fall of the co-worker by reaching out and using his arms and caused strain to his left arm and shoulder area. Injured employee was taken to Urgent Care and diagnosed with a supraspinatus tear in left shoulder.
Employee was attempting to ascend steps on at coal load-out area and slipped causing right leg to twist. Employee was taken to local hospital for observation and x-ray and was diagnosed with a medial meniscus tear and put on restrictive duty until further notice.

While attempting to cut bolt heads off screen deck frame, the 4-1/2" hand held grinder jumped and cutting wheel touched employees left thumb causing a laceration. Employee was observed at local hospital and received three stitches.
Employee was attempting to remove worn feed end top deck screen from Unit #657 vibrator. While employee was pulling, the screen got stuck and caught on a side piece of angle iron. This caused a strain to employee's lower back area. Employee was observed at local hospital and diagnosed with a lumbar strain.
Employee was running to put windows up in pick-up truck due to driving rain. While attempting to run around the rear of the truck the employee's forehead struck an aluminum extension ladder. Employee was taken to hospital for observation and received 8 stitches.
Employee received a strain to lower back area when the Komatsu WA450-5L wheel loader he was operating hit a pot hole causing sudden jarring. Employee was taken to hospital for observation, given a steroid injection, then returned to work with no restrictions.

Employee stepped over guard rail on this day and twisted ankle. No medical attention was sought. On 11/11/13 employee received surgery for same right foot and has been awarded and receiving workers compensation.
While walking around the Cat TL943 Telehandler with intentions to enter the operator's cab, employee slipped on ice causing a fall. The employees right ankle and foot twisted and landed underneath his body. This caused pain and soreness to employees right ankle area. Employee was taken to local hospital for observation and diagnosed with a fracture to the right ankle.

While performing maintenance, employee was inspecting shaft on head wheel of conveyor for burrs and rubbed hand across the surface of the steel shaft. A sharp burr caused a laceration to his RIGHT index finger requiring eight (8) stitches.
Employee received an insect bite, believed to be from a spider. After noticing a mark and red streak employee went to hospital to have bite observed by a professional. Employee was given antibiotics, tetanus shot and released back to work.
Employee was attempting to cut a piece of metal rectangular tubing off the portable diesel pump with 4-1/2" grinder. The grinder bound, kicked back and cutting wheel caused laceration to left hand area between index and middle finger.
Employee was performing maintenance to adjust belt scraper, and he slipped with his LEFT leg striking steel sideboard of chute causing pain and a cut on back of his leg behind the LEFT knee.

Employee was pulling fuel hose and nozzle off rear reel of XSV2 service truck attempting to fuel WA600-6 Komatsu loader. While pulling fuel hose employee felt a pain and soreness in his right shoulder.
While lifting 10" worn piece of steel pipe onto work table employee grabbed the flanged end, began to lift and his left hand slid along worn inside section of pipe causing cut to work gloves and penetrating through and cutting left ring finger.
While employee was attempting to clear a foreign object (metal loader tooth) from the bottom deck of the McClosky portable screener he reached forward and tried to lift the foreign object causing pain and tightness in middle section of lower back.
Employee was standing on middle of stairway, turned around to go back up and felt a pop and strained his left knee.
While working main entrance employee felt lightheaded and fainted. Co-worker checked on him and found lying on ground. EE called Operator employee for assistance and they called 911. Paramedic' s responded and transported EE to local hospital for observation.
A security employee was exiting an office trailer at the mine site of Lehigh Natural Resources water impoundment location when he rolled his ankle on the last step of the stairs, the employee was driven to Saint Lukes Hosptial,Coaldale,P.A. where he was attended to by an E.R. doctor who made a visual exam & ordered x-rays & determined he had a minor sprained ankle

On Thurs 4/3 the employee notified employer of a personal day for Friday 4/4. On Friday morning at 0800 employee called and said ankle was swollen, and would like to see doctor. At this time claim was opened.

Employee was working on the repair of equipment. Lifting a gear, he went off balance, cut finger, pulled back.
While lifting a 5 gallon pail of oil to the back of a pickup truck, EE felt a pulling sensation in his right shoulder.
Employee was standing on a cross brace approximately 12" off the ground when he slipped off the bracing. He attempted to steady himself by grabbing an above pipe subsequently strained his left shoulder when his full body shock load was realized. Employee was not at work on 5/10 as he was sent to company doctor for further evaluation and returned to work on 5/11.

EE WAS RETURNING SOME MATERIAL TO A STORAGE SHELF. WHEN THE EE LIFTED A ROLL OF RED RUBBER GASKET MATERIAL ONTO A STORAGE SHELF, HE FELT A PAIN IN HIS RIGHT ARM/SHOULDER AREA. THE ROLL WEIGHED LESS THAN 100 LBS.
ON 12-20-95 AT APPROX. 23:10 HRS., I.NAME DISCOVERED A FIRE BURNING IN THE BUILDING COMMONLY CALLED THE HEADHOUSE. HE ACTIVATED THE EMERGENCY ACTION PLAN IN EFFECT AT THE TIME AND AWAITED THE FIRE COMPANIES TO ARRIVE TO EXTINGUISH THE FIRE. THE BUILDING SUFFERED EXTENSIVE DAMAGES. THERE WERE NO REPORTED INJURIES. THE FIRE WAS DECLARED OUT IN TWO HOURS.
DURING NORMAL REPAIRS, THE EMPLOYEE WAS TIGHTENING A BOLT WITH A RATCHET WRENCH WHEN THE WRENCH SLIPPED OFF THE BOLT. THE EMPLOYEE STRUCK AGAINST AN ANGLE IRON, CAUSING AN AVULSION TO HIS RIGH T RING FINGER.
EMPLOYEE WAS ATTENDING TO COAL STOVE. WHEN HE OPENED THE DOOR ON THE STOVE TO PERFORM REGULAR INSPECTION OF THE FIRE, THE AIR CAUSED THE FIRE TO IGNITE, FLASHING UPWARD INTO THE EMPLOYEE'S FAC E. EMPLOYEE DID NOT KEEP HIS FACE IN A SAFE POSITION WHEN OPENING THE DOOR.
EE WAS REPAIRING A HAND PUMP. THE PUMP SLIPPED FROM THE VICE & FELL TO THE GROUND, STIKING THE EE ON THE LEFT FOOT.
EE WAS LIFTING AN 8" CHANNEL INTO POSITION DURING A REPAIR WHEN HE FELT A PINCH IN HIS BAKC. THE PAIN SEEMED TO RADIATE DOWN HIS LEFT BUTTOCKS AND LEG.
EMPLOYEE WAS SEARCHING FOR A PIPE WHEN SHE FELL OVER SOME DEBRIS, FALLING TO THE GROUND AND LANDING ON HER LEFT ELBOW. IT WAS A DARK AREA WHERE SHE WAS WALKING.
EMP SLIPPED ON WET SURFACE OF HIS TRUCK CAB WHILE CHECKING FLUID LEVELS.
EMP STRAINED HIS LEFT ELBOW AREA WHILE SHOVELINGSPILLED MATERIALS AND WORKING WITH A WHEELBARROW
EMPLOYEE WAS GRINDING A PUMP CASTING WHEN THE GRINDER DEFLECTED OFF THE PUMP, STRIKING THE EMPLOYEE IN THE LEFT LEG.
DURING REPAIRS TO THE RIPPLE-FLOW VIBRATOR, THE EE WAS STRUCK IN THE RT EYE WITH SOME FOREIGN MATERIAL WHEN THE WRENCH HE WAS WORKING WITH FELL TO THE FLOOR, CHIPPING DEBRIS INTO THE AIR.
EE WAS PERFORMING REPAIRS TO THE 5 VIBRATOR WHEN HE WAS BIT ON THE RIGHT LEG BY A SPIDER.
EMPLOYEE WAS LIFTING A STEEL PLATE INTO POSITION IN THE RAW COAL HOPPER WHEN HE FELT A PAIN IN HIS BACK EMPLOYEE STATES HE MISJUDGED THE WEIGHT OF THE PLATE
EMPLOYEE WAS FUELIGN HIS TRUCK AT THE 111 FUEL RACK. WHEN THE EMPLOYEE WAS STEPPING FROM THE PLATFORM HIS FOOT SLIPPED ON THE FUEL COVERED GRATING, CAUSING HIM TO FALL, LANDING ON HIS BUTTOCKS .
EE WAS INSTALLING A NEW CONVEYOR LINE IN THE HEADHOUSE. EE HAD HIS LEFT FOOT CAUGHT BETWEEN THE CONVEYOR AND A SUPPORT BEAM WHEN THE CONVEYOR MOVED UNEXPECTEDLY.
WHILE REPLACING A SCREEN PATCH ON THE VIBRATOR, EMPLOYEE WAS STRUCK IN THE RIGHT EYE WITH A FOREIGN BODY.
EMPLOYEE WAS PERFORMING MAINTENANCE ON THE SOUTHEGG CAMS EMPLOYEE STOOD ON A CAM TO BETTER POSITION HIMSELF WHEN HE FOOT SLIPPED, CAUSING HIM TOOVEREXERT HIS RIGHT KNEE. HE STATES HE HAD GREAS E ON HIS SHOES.
WHILE CLEANING OFF A SUPPORT AREA NEXT TO AN IMPOUNDMENT POND WITH A FRONT END LOADER, THE OPERATOR WAS JARRED WHEN THE LOADER STRUCK A ROCK HIDDEN UNDER SOME MATERIAL. THE OPERATOR WAS WEARING A SEAT BELT.
