Lehigh Anthracite Coal, LLC · Caught in, under or between a moving and a stationary object

While assisting welder to pull tooth out of metal bin for 6015B bucket, tooth slipped pinching employees right index finger against metal bin. employee was taken to local urgent care for observation and diagnosed with laceration to right index finger.

Lehigh Anthracite Coal, LLC · Fall from ladders

Performing maintenance greasing portable screening plant, employee descended fixed ladder, slipped and slid ~4' down metal steps of ladder scraping belly and stomach area against steps, landing on feet. Employee refused treatment. Two days later, employee was sore, went to Urgent Care for observation and diagnosed with two fracture ribs with restricted duty.

On 1/19/22 while attempting to ascend truck, employee placed one foot on ladder, planted other foot, slipped on ice. EE then reached and grabbed hold of railing to prevent fall and hurt left shoulder area. Two weeks later EE's left shoulder/neck area was still sore. 2/02/22 EE was taken to Urgent Care for observation and diagnosed w/muscle strain.

Employee slipped and fell on ice at blast site. Did not seek immediate medical attention. Discomfort in left shoulder persisted. Diagnostic screening did not result in restrictions until 3/6/19. Originally reported to MSHA on 3/6/19. Report not found on-line. Resubmitting 8/15/19.

Driving haul truck, being loaded by an excavator, a rock fell off the bucket as it moved over the truck, hitting the left rear corner of the box causing the operator to spring forward and then back in the cab, possible whiplash.

On Saturday, July 21, 2007, the victim, while trespassing on mine property, was operating a 4-wheel recreational vehicle and traveling on the mine haul road. He lost control of the vehicle resulting in serious injuries. The victim died of his injuries on July 25, 2007.
