EE WAS CHECKING WATER PUMP DISCHARGE LINE. GROUND WAS WET IN AREA & HE SLIPPED & LANDED AGAINST A LOG HITTING HIM IN THE RIB CAGE AREA. HE APPEAR FINE, BUT A FEW DAY LATER A BRUISE APPEARED & HE HAD IT CHECK BY A DOCTOR.

EMPLOYEE IS A HEAVY EQUIPMENT MECHANIC & HE USES HIS HANDS DAILY WHICH CAUSED HIM TO DEVELOPED CARPAL TUNNEL IN HIS LEFT & RIGHT HAND. SURGERY WAS DONE TO CORRECT THE CONDITION. AS A RESULT OF A PART 50 AUDIT DONE 4/12/93 IT WAS DECIDED THAT THIS ILLNES HAD TO BE REPORTED ON THIS FORM - 7000-1

HE WAS GIVING HAND SIGNALS TO I.NAME BACKING A MECHANICS TRUCK UP TO A DRILL TO MAKE REPAIRS. HE WAS RIDING ON THE REAR STEP BUMPER. HE FAILED TO SEE THE LID OF A TOOL BOX ON THE DRILL WAS OPE N. THE TRUCK BACKED INTO THE TOOL BOX LID HITTING HIS LEG CLOSE TO THE ANKLE BREAKING THE SMALL BONE IN HIS LEFT LEG.
