The employee was utilizing a torque multiplier tool to remove a tie rod form a hydraulic rock breaker and while releasing tension on the multiplier the ratchet that EE was using broke a tooth, skipped and quickly spun around and caught the employee's hand and caused a laceration.

On Tuesday 2/14 around 7:10 am EE was rolling up a 24 inch wide conveyor belt when EE slipped and fell, injuring lower back. This incident was not reported until Wednesday 2/15 by text messaging Supervisor at 2:27 am. Supervisor called and informed me about the incident.

While attempting to move from under the machine, our Technician hit hard hat on the step located on the side of the machine knocking off hard hat resulting in a scalp laceration requiring stitches.

As he was elevating the man lift up under a conveyor, the machine cover on the man lift came too close to an "I-Beam" and his hand got caught between the control panel and the underside of the I-Beam. He lacerated his hand requiring 5 stitches and has hairline fractures on 2 fingers. Right Hand.

Employee was injured when he was replacing the side guard on a loader bucket. The mechanic had one of the bolts in place and was using a 4 lb hammer to hit the side guard in order to align the other hole. The hammer struck the side guard and a steel chip flew off and struck his right inner thigh. The piece of steel went through his clothing and had to be surgically removed.
