I CLIMBED UP ON THE TRUCK BED TO LEVEL LOAD DOWN AFTER THE TRUCK WAS LOADED AND SLIPPED AND FELL FROM THE BACK OF THE TRUCK LANDING ON THE LEFT SIDE OF BACK AND RIBS, CAUSING A BRUISE TO SAME TRUCK WAS ON ROUTE TO MAIN HIGHWAY FROM ACCESS ROAD TO FIELD STONE AREA.

A STAND UP BATTERY CHARGER WAS HOOKED UP TO START A JOHN DEER FORKLIFT. WHEN THE CONNECTIONS WERE MADE, THE EMPLOYEE REACHED ACROSS THE STEERING WHEEL TO START THE MACHINE, THINING SAME WAS OU T OF GEAR. THE FORKLIFT WENT INTO REVERSE, KNOCKING THE BATTERY CHARGER INTO HIM CAUSING HIM TO GO TO THE GROUND WITH THE CHARGE LANDING BETWEEN HIS LEGS CAUSING A BREAK TO THE PELVIS BONE.
