Cullum Equipment Services, Inc · Struck by falling object

3 employees were changing 2 worn Jaw Dies. After successfully removing both and replacing one, hooked up second jaw die to lift in place when chain hook slipped out of lifting eye and jaw die overbalanced and fell on one miner's leg. The 2 supervisors quickly used crow bar to free miner. The miner was immediately taken to the ER and it was discovered his knee and leg was broken.

Employee was raising the leg of a lean-to shed with a cable come-a-long to place pipe under leg for a reinforcement brace. Pipe slipped and mashed thumb.
