Employee had a new on-road dump truck tire up against the work bench, employee was bent over to put cap on and tire popped off the rim, hitting the work bench and then came at employee hurting employee by forcing employee to the ground causing broken right forearm, wrist and thumb bones, with exposed bone on wrist. Also broken right tibia in leg with exposed bone.

Changing the battery of the plant generator,(11 degrees out). The wrench employee was using, slipped out of hands, hitting the battery causing it to arc and explode. Wearing safety glasses at the time. Battery acid splashed up and some went into right eye causing a chemical burn.

Tooth from drag line came off and was stuck in crusher. Turned off crusher, climbed up to crusher to free tooth...popped up and hit on right hand, middle finger/knuckle. Laceration to knuckle/middle right finger. Had to go to ER for stitches to stop bleeding.

Employee was out working on the plant platform of the screen deck. He bent down to pick up something, when he came back up he hit his head on a steel beam. There was a small gash on top of his head that needed 4 staples to close.
