CONTRACT OPERATOR OF MACKR700 TRRACTOR TRAILER TRUCK WAS LOADED WITH COAL & ATTEMPTED TO LEAVE SURFACE MINE PIT. TRUCK WAS HUNG UP IN WET PIT & COULD NOT GET OUT. UE TO DESIGN OF TRUCK CABLE C OULD NOT BE USED TO PULL TRUCK OUT & 1/2" LINK CHAIN WAS ATTACHED TO TRUCK & KOMOTSU D-41A DOZER. AS DOZER ATTEMPTED TO PULL TRUCK FREE CHAIN BROKE WITH PIECE OF CHAIN STRIKING INJ.

THE ACCIDENT VICTIM, A CONTRACTOR EMPLOYED BY DULANEY OIL COMPANY, DENTS RUN BLVD, WESTOVER, WV, HAD COMPLETED THE UNLOADING OF DIESEL FUEL. THE DRIVER STEPPED INTO HIS TRUCK AND ACCIDENTLY RE LEASED THE EMERGENCY BRAKE. DUE TO THE FACT THE TRUCK WAS IN NEUTRAL AND PARKED ON A GRADE, THE TRUCK PROCEEDED TO ROLL BACKWARDS. THE OPERATOR JUMPED FROM THE TRUCK WHEN IT STARTED TO ROLL B
