EE WAS USING A SHORT BAR TO PUSH IN A SWEEP. THE BELT CLIPS CAUGHT THE BAR AND PULLED IN HIS FINGER. FINGER CAUGHT BETWEEN THE CONVEYOR BELT AND FRAME. EE FAILED TO LOCK-OUT EQUIPMENT. (AMPUTATION LEFT INDEX FINGER - DISTAL PHALANGE.)

EE WAS INSPECTING A DAMAGED 480V POWER RECEPTACLE THAT HAD FLASHED. DURING HIS INSPECTION HE PICKED UP THE RECEPTACLE & IT FLASHED AGAIN . EE FAILED TO DISCONNET POWER TO RECEPTECLE PRIOR TO HIS INSPECTION
