EE was pulling rope through 8" pipe which was attached to a 5" hose. The end of the pipe 4.5' from edge. EE was near the face of a 15' drop. EE should've been tied off to prevent falling. EE wasn't. Rope broke & EE fell backwards & landed on feet & hurt left foot. EE had bruised hand w/minor cuts. Brought EE to ER, x-rays determined fractured heel. EE was wearing hard hat.

Employee was getting into Cat truck. Opened door, went to step in and missed the catwalk and fell about 6 feet. 4 stitches above right eyebrow, soreness to shoulder, arm and ribs.

AFTER THE EMPLOYEE HAD JUST COMPLETED DRILLING HOLE IN FACE, HE MOVED DRILL AND RELEASED HIS SAFETY LINE TO INSERT CONE IN HOLE. AS HE WAS INSERTING CONE, THE AREA HE WAS ON GAVE WAY AND HE SL IPPED 12 FEET AND THEN FELL ANOTHER 12 FEET TO GROUND. HE RECEIVED LACERATIONS, BROKEN RIBS AND BROKEN FOOT.
