While using a cut off saw to remove a collar from a hose, a piece of metal got around safety glasses and impacted miners left eye.

Shaker on plant was locked up, ee was checking vee belts to see if they were tight or not. the counterweight rolled down, and lodged ee's hand between vee belts and the sheave. the ee's left hand received a laceration on his 1st finger requiring 5 stitches, and a severe bruise on his 2nd finger.

TWO MEN WERE PRIMING THE PUMP ON THE PONTOON. THEY BOTH STOOD ON THE SAME SIDE CAUSING THE PONTOON TO FLIP. THEY LANDED ON THE ROCKS IN THE WATER.
