Miner was attempting to relocate a 4" pump discharge line when EE injured their lower back. Later determined to be a muscle strain.

Maintenance Tech was performing maintenance on a D-8 Bulldozer. Miner slipped when trying to cross over the blade to add oil and fell on the blade brace. Miner injured left knee from the fall.

Miner had just completed adding oil to the Hitachi ZX450 excavator when EE stepped down onto the track. After stepping down onto the track. Then, slipped on the track, fell against the catwalk of the machine. EE completed EE's entire shift. It was not until the end of shift EE noted pain in EE's rib area. EE did not report the accident until the following morning (5/6/2022).

LP Mineral LLC · Struck against a moving object

Driver said EE was empty going towards coal load out when EE hit a pot hole in the road and hurt EE's back. Went to Dr. and took X-ray and found out EE had a compressed disk. Dr. had EE off three days or light duty work. Returned to driving on 12/7/2017 with no restrictions.

Contractor was feeding brush into burn pile. Operator said he heard something pop. Swung his excavator around to pick up another log and saw fire. He trammed away from burning pile. Not sure if hose blew or a hot cinder started fire. Name of contractor - Gibson Reclamation. Fire lasted about an hour. Granille Fire Dept. assisted.
