EMPLOYEE WAS OPERATING THE STAWARD SIDE OF A DOUBLE HEAD BOLTER. THE OFFSIDE OPERATOR DRILL STEEL STOPED UP AND HE WENT OVER TO THE OFFSIDE OF THE MACHINE TO ASSES HIM. THE STEEL WAS UNSTOPED THE BOLTER HEAD WAS RAISED AND CAUGHT EMPLOYEE\S HAND IN THE HEAD.

MURDOCK WAS CLEANING OFF THE TOP OF THE COAL BREAKER AND JUMPED ABOUT 10 TO 15 FT TO THE SURVACE GREG SAID HE FELL TOT HE SURFACE LANDING ON BOTH FEET HE SAID HIS KNEE GAVE WAY AND HE CON CONTINUED TO FALL TO HIS KNEES GREAG SAID HE TRIED TO WALK AND HIS LEFT KNEE WOULD NOT STAY IN PLACE
