Employee was turning off/on an electrical box when the box arced internally causing the latched door to swing open. The door hinges at the top of the box; when it swung open it hit EE in the bridge of his nose. He was not electrocuted but there was an electrical flash. EE was bleeding from a cut on his nose and was taken to hospital for examination.

Illinois Mining Corporation · Struck by falling object

Employee blocked and jacked the welding truck to work on it. While working under it, he called over a co-worker to assist. A third employee was also asked to help and while working on the welding truck, it swayed sideways, causing the jack and blocks to roll, placing the truck down onto its tires. The employee under the truck sustained bruises, a fractured collarbone and pelvis.
